Job summary

The Wound Care nurse will work as part of the primary healthcare team providing wound assessment and dressing services to the practice patients, therefore, previous experience in this area of expertise is essential.

Main duties of the job

Assess, review and completewound care for patients including but not limited to:

-Post Operative (clip and suture removal)

-Cavity Wounds

-Burns

-Self Harm

-Skin Tear

-Cellulitis

-Referral to the wider Multi-Disciplinary Teams

Provide a plan of care usingwound care policies and pathways for guidance utilising the wound care dressingformulary

Liaise with other ClinicalStaff when indicated

Provide holistic assessment ofpatients needs making onward referrals where required

Other tasks will include

-Maintain general tidiness and cleanliness ofnurses and treatment rooms, including stocking and rotating items as required

-Maintaining and cleaning equipment used by thenurses

-Participation in administrative systems in thePractice

-At all times there will be a need to maintainaccurate records

-Attend and participate in any Practice meetingswhen required

-Any other delegated duties appropriate to thepost

About us

Dr Shimmins & Partners have 6 GP Partners, 2 Salaried GP's, Practice Nurses, Healthcare Assistants, Clinical Pharmacists, Paramedics and other associated clinical roles assisted by 2 Managers and and a large number of administrators

We are located on East side of Dartford with good transport links including trains, motorways and are close to Darent Valley Hospital.

We are a GP training practice and belong to Dartford Model PCN.

We have approximately 15,600 registered patients and serve a wide catchment area.

Person Specification

Qualifications

Essential

Registered NMC nurse with clinical supervision training and experience.

Desirable

Relevant nursing/health degree. Wound Care qualification.

Experience

Essential

Experience of working within a nursing team. Experience of dealing with vulnerable patients.

Desirable

Experience of working within the community as a wound care nurse. Experience of working in a GP practice.
