Main duties of the job

You will be required to undertake home visits to administer treatments and clinically assess patients . You should have the ability to think on the spot and problem solve and recognise when to escalate to the team.

Detailed job description and main responsibilities

Clinical Responsibilities:

Act as a self-directed practitioner who directs, provides and evaluates the highest standard of evidence-based nursing care to patients within their own home, founded on agreed management care plans and protocols. 

Work independently and without direct supervision in a variety of clinical settings, Trust wide and in the community, prioritising workload to meet the regular unpredictable situations that may occur within this role.

Demonstrate the ability to work independently and perform comprehensive patient assessments based upon history taking and physical examination.

Demonstrate highly developed clinical reasoning and decision-making skills.

Make and receive direct referrals to and from the relevant members of the multi-professional teams.

Acquire a variety of specialist nursing skills and knowledge, including assessments, planning and management of chronic diseases in the home, such as heart failure, wound management and respiratory disease.

Education and Training

Essential criteria

Degree/Diploma in Adult Nursing RGN Level 1 ENB 998 or equivalent Assessor/Mentorship course Post Registration training in chronic disease management in acute or community setting Basic Life Support Cannulation/phlebotomy Evidence of CPD

Desirable criteria

Respiratory Course Cardiac Course Enhanced Clinical Assessment Course NVQ Assessor

Knowledge and Skills

Ability to analyse professional and ethical issues Working knowledge of national policies and procedures across Health, Education, Social Care and its implementation or application Ability to organise and respond efficiently Understanding of team dynamics and ability to work collaboratively within a multi-disciplinary team Effective written and oral communication skills Ability to work autonomously and set priorities Ability to reflect and appraise own performance Training and facilitation skills Presentation skills Ability to build effective working relationships Awareness of Health and Safety risk Ability to travel to destination in a timely manner.
