Job summary

New Islington Medical Practice is a large GP surgery based, servicing the needs of over 11000 patients in Manchester City Centre in the newly developed Ancoats area.

We are looking for an enthusiastic and experienced Practice Nurse to assist in the management of our Chronic Disease Patients including Diabetes, Asthma and COPD. Cervical Smears and Childhood Immunisations are also a necessity, however ongoing training can be given to the right candidate.

Main duties of the job

We are looking for a dedicated practice nurse to join our close knit practice team. Training can be provided for those candidates who do not have a current full skill set.

A sense of work enjoyment and sense of humor is a definite must.

The practice offers a Digital First approach to all services we provide and the right candidate should have basic computer skills which can be developed.

Child Immunisations, Travel vaccine, Cervical Screening and Chronic Disease Management is a requirement for this role. (update training can be given)

About us

We are a multi disciplinary team made up of a variety of clinicians including GPs, nurse practitioners, HCA, phlebotomist, training GPST and FY2 doctors, physiotherapists, mental health nurse and practice based pharmacists. Our admin and reception team round things off, with 2 managers working alongside our GP partners.

We're a close knit team made up of staff taken all the way from their apprentice days into fully trained positions and also staff members who have joined us in the past year.

Working in the vibrant City Centre, our patient population is growing daily and we have quite a mix of patient needs.

Jobsummary:

The post holder will work towardscarrying out the full range of duties below, with appropriate training andsupport to develop competence and build the required skills.

The role is to provide a practice nursing service to patients,including chronic disease management and other specialist areas whilst takingappropriate initiative and working independently.

The post holder works as part of the clinical team to meet theneeds of patients.

The Practice has a commitment to excellent patient care, qualityimprovement and teaching. It is expected that all of our clinicians areactively committed to supporting these aims and participate in relevantactivities when appropriate.

Jobresponsibilities:

Following Practice policies, protocols and guidelines, duties willinclude:

Responsibilities for patient care

Assess the needs of patients, providing appropriate care/treatmentin conjunction with the GP, or independently as appropriate.

Refer to colleagues or other agencies as appropriate.

Provide the full range of nursing services, includingvenepuncture, ECG measurement, blood pressure measurement, urinalysis,vaccinations and immunisations (children and adults including travel), baby clinic,injections, cervical smears, swabs, travel services, health checks, assessmentof risk factors, lifestyle advice, weight management, smoking cessation, suture removal, family planning, etc.

Manage the care of patients with long term conditions. This mayinclude organisational responsibility for specific disease areas.

Assess, plan, implement and evaluate treatment plans for patientswith long term conditions (or at risk of developing one) that meet identifiedneeds and promote health and well-being.

Select appropriate tests for patients, and interpret and act onresults where appropriate.

Support effective treatment and prescribing by working withpatients in order to support adherence to prescribed treatments, reviewing medicationfor therapeutic effectiveness, and providing information and advice (includingover-the-counter medication) on medication regimens, side-effects andinteractions.

Educate and support patients as needed, including adopting healthylifestyles, appropriate self-care for minor illness and following care plansfor long term conditions.

Assist in minor surgery procedures and IUD/implant fitting when required. (Training will be provided)

Priorities health problems and interveneappropriately to assist the patient in complex, urgent or emergency situations,including initiation of effective emergency care.

Organise and/or run initiatives, includingcontacting patients, to improve uptake of screening, vaccinations or otherservices and support the achievement of targets ( under the Quality andOutcomes Framework).

Maintain accurate and contemporaneous records torelevant professional standards/guidelines.

Delivery of the patient service may requirevisiting patients at home or occasional weekend working. In particular thepostholder will be expected to take part in annual flu vaccinations outsidenormal hours

Teaching and Training

Take responsibility for own development, learningand performance including participating in clinical supervision and training asnecessary to update skills and knowledge and maintaining a record of ownpersonal development.

Receive review and appraisal within the Practice.

Participate in the educational and trainingevents of the Practice.

Organisation and delivery of the appropriateparts of educational programmes for medical and nursing students, GP Registrarsand other learners, taking the lead and/or directly supervising as required.

Participate in the induction of new staff,students and other attached professionals as necessary.

Contribute to and deliver in-house trainingsessions for clinical and non-clinical staff.

Provide training to other staff where relevant tothe post.

Feed back training or other information to teamwhere appropriate.

Provide mentorship of staff in appropriatecircumstances.

Ensure clear understanding of and appropriateutilisation of services and referral
