Job summary
We are excited to offer this opportunity for an experienced or newly qualified, health visitor to join our unique and innovative Specialist Health Visiting Service. Health Visitors in Portsmouth TEDS (Targeted Early Development Support) Team lead in the delivery of the Healthy Child Programme, HCP, alongside supporting families with Children aged 0 to 5 years with multiple health needs, developmental delay and or a disability.
The successful candidates will be proactive and enthusiastic individuals who are looking to further develop their public health nursing skills to deliver a quality evidence-based service to families. Applicants will be inspirational and motivational leaders, highly skilled in communication, with a strong approach to teamwork. As a specialist Health Visitor you will take the Lead professional role and work alongside other community health services, including paediatricians, therapists and community nurses.
We are an integrated 0 to19 service with the Local Authority and work with the wider multi-agency and multi-disciplinary children's workforce to identify need and provide timely intervention at the earliest opportunity. Health visitors are well supported to achieve this through a developing Team Around the Worker model, utilising consultation with other specialist practitioners working across the 0-19 service in areas such as perinatal infant mental health, substance misuse, domestic abuse and children with disabilities.
Main duties of the job
The post holder will work within the Targeted Early Development Support Team, TEDS Team. You will be supported in your role by the Clinical Team Co-ordinator for TEDS. You will deliver an enhanced health visiting service to families who have children with multiple health needs, development delay and/or a disability. You will work with babies and children aged 0-5 years, who are registered with a Portsmouth GP. You will take the lead professional role and work alongside other community health services including paediatricians, therapists, and community nurses. You will provide Early Help, signposting to other services and targeted interventions tailored to the individual child's needs. You will ensure co-production with parents and education. You will be providing updates to the Early Years Support Panel, liaising with nurseries, and contributing to Education Health Care needs assessments. To ensure that safeguarding is a key priority within work with children and families To act as a clinical leader and health expert ensuring evidence-based practice is delivered to improve and assure the quality of care and to shape responsive services to meet identified need.. Accountable for the delivery of the Healthy Child Programme within a designated caseload, working with SystmOne as clinical record and work planner. To identify and provide targeted services to vulnerable children and families based on the Family Support Plan, FSP.
