Job summary

We are delighted to offer an exciting opportunity for a Band 7 Physiotherapist to join our friendly, dynamic and highly regarded community based locality rehabilitation teams.

The community rehabilitation teams are a key partner in supporting Discharge To Assess, Virtual Wards, Admission Avoidance and Home Based Intermediate Care teams and work with people in their own homes ensuring they receive the appropriate specialist long term rehabilitation to reach their goals and recover.

This post is part of a wider Integrated Care Directorate to support the patient and their family to keep them safe at home. We work together to improve clinical outcomes, start timely rehab to prevent deconditioning and provide the intensity of intervention to maximise their quality of life.

As a member of the leadership team you will be expected to role model excellent patient care, have oversight of the Community pathways and work with our partners to deliver the best integrated service.

Main duties of the job

The key duties for the post holder will be;

To provide leadership and oversight, as a Community Band 7 Physiotherapist, working within an Integrated team following a range of clinical pathways. A key function of this role is to work collaboratively with our local partners (local authority reablement team/ social services/ third sector - like charities) and the local people.

You will work closely with your Band 7 peers, and the iCares Service Leads along with the wider iCares Service.

To provide highly developed specialist assessment and treatment of patients with planned as well as unplanned complex presentations within community settings.

To supervise, assess and educate team members including bank or agency staff, support workers and students.

Within iCares we operate a 7 day service 8am-8pm, we all contribute to weekend working and 8pm finishes.

Candidate must hold a valid UK driving licence and have a vehicle with business insurance which they can use for work purposes.

Mileage can be claimed for journeys at the usual NHS rate.

Degree/Diploma in Physiotherapy HCPC Registration

Experience

Essential

Education of junior staff or under-graduate Physiotherapy students Working as part of a Physiotherapy team and the wider MDT Supervision and delegation of work to other Therapists and support staff Sufficient experience at undergraduate and post graduate level to meet the elements of the Job Description. Participation in emergency duties

Desirable

Staff development and management of informal disciplinary matters Involvement in audit & research Policy/protocol development/ evaluation

Leadership

Organise and prioritise own and team workload Line manage and deploy staff effectively to meet service needs Able to respond to interruptions in work patterns in response to service needs and Trust discharge deadlines and in response to bleeps. Able to cope working in a stressful, hostile or highly emotive environment with patients and carers of patients presenting with disabilities, chronic or terminal conditions, including those with limited recovery potential. Able to deal sensitively with patients with high levels of anxiety/aggression caused by pain, dementia, sedation, limited mobility or who are unable to accept rehabilitation outcomes. Able to deal with situations involving conflict and dispute and disciplinary matters
