Position Overview

We are pleased to present a remarkable opportunity for a Band 7 Community Rehabilitation Physiotherapist to become a vital part of our esteemed and supportive locality rehabilitation teams.

The community rehabilitation teams play a crucial role in facilitating Discharge To Assess, Virtual Wards, Admission Avoidance, and Home-Based Intermediate Care services. Our focus is on delivering specialized long-term rehabilitation to individuals in their residences, enabling them to achieve their personal recovery goals.

This role is integral to a broader Integrated Care Directorate, dedicated to assisting patients and their families in maintaining safety and well-being at home. Our collaborative efforts aim to enhance clinical outcomes, initiate timely rehabilitation to prevent deconditioning, and provide intensive interventions to elevate the quality of life for our patients.

As a key member of the leadership team, you will exemplify outstanding patient care, oversee community pathways, and collaborate with our partners to provide the highest standard of integrated services.

Main Responsibilities

The primary responsibilities for the successful candidate will include:

  • Providing leadership and oversight as a Community Band 7 Physiotherapist within an Integrated team, following various clinical pathways.
  • Collaborating closely with local partners, including local authority reablement teams, social services, and third-sector organizations.
  • Conducting highly specialized assessments and treatments for patients with both planned and unplanned complex presentations in community settings.
  • Supervising, assessing, and educating team members, including bank or agency staff, support workers, and students.

Service Commitment

Within iCares, we maintain a 7-day service from 8 am to 8 pm, with all team members contributing to weekend shifts and evening finishes.

Candidate Requirements

Applicants must possess a valid UK driving license and have access to a vehicle with business insurance for work-related travel.

Compensation

Mileage for work-related journeys can be claimed at the standard NHS rate.
