Job summary

Thepost holder is an experienced Nurse with a minimum of two years experienceworking as an autonomous practitioner. They should be working within theirprofessional boundaries and will provide care for the patients within both theirown homes and care homes as part of an integrated team within practices.

Theywill be skilled in initial history taking, clinical assessment, diagnosis,treatment and evaluation of care; whilst demonstrating a safe, criticalthinking clinical decision-making process and expert care delivery for thosepatients who are vulnerable and at risk of avoidable admissions within the generalpractice population.

Thepost holder will be integrated into practice teams, whilst working collaborativelywith the general practice team members, care home staff, patients, and otherHealthcare professionals to meet the needs of this group of vulnerable patients,whilst supporting the delivery of best practice, policy and procedures.

In order to work at this level NMC requirementsfor advanced practice must be met.

Main duties of the job

Clinically examine and assess patient needs from a physiological and psychological perspective, and plan clinical care accordingly Assess, diagnose, plan, implement and evaluate treatment/interventions and care for registered patients presenting with an undifferentiated diagnosis and long-term chronic conditions Assess, diagnosis, plan, implement and evaluate interventions/treatments for patients with complex needs Proactively identify, diagnose and manage treatment plans for patients at risk of developing a long-term condition (as appropriate) Diagnose and manage both acute and chronic conditions, integrating both drug- and non-drug-based treatment methods into a management plan Prescribe and review medication for therapeutic effectiveness, appropriate to patient needs and in accordance with evidence-based practice and national and practice protocols, and within scope of practice Work with patients in order to support compliance with and adherence to prescribed treatments Provide information and advice on prescribed or over-the-counter medication on medication regimens, side-effects and interactions

Job responsibilities

Work within the IntraHealth federation to achieve best health outcomes and where possible prevent unplanned hospital admissions.

Clinical governance issues in primary care Independent non-medical prescribing qualification

Relevant nursing/health MSc / Degree in Advanced Nursing Practice.
