 Ty Garngoch Secondary care service is offering a hugely exciting opportunity to be part of a supportive, forward thinking, adaptive team.

We are seeking an enthusiastic Rmn Clinical lead to join our dedicated and dynamic multidisciplinary team to support our Older Peoples Mental Health Service (OPMHS) in Swansea covering area 3 +4.

Our service provided community care services for older adults (over 70) with functional mental health illness and complex health issues. We treat frail patients with complex care needs and a wide range of functional problems. We provide treatment care and support for people with dementia.

The vision for OPMHS is to offer a holistic approach to care, which focuses on assessment of functional and organic mental health problems for older people. Our aim is to provide treatment and rehabilitation for those with chronic disabilities, including adequate and appropriate facilities for the care of patients to optimise their mental and physical health.

Main duties of the job

The candidate will demonstrate an understanding of clinical interventions relevant to Older People to include 
education, care plans and discharge planning, working closely with carers, families and partner agencies 
to facilitate this. The post holder will work at all times to promote the safety and the wellbeing of service 
users and their families/carers. The post holder will assess, plan and implement care, and provide 
specialist nursing advice and carry out specialist nursing procedures. They will provide clinical 
supervision to staff and students.

Detailed job description and main responsibilities

The post- holder will be responsiblefor supporting the Team Manager in the day-to-day clinical management of a designated older peoples’ Secondary Care Community Mental Health Assessment and Treatment Team within an identified geographical area, ensuring that nursing care is delivered to the highest standard. The post holder will Care-Coordinate a caseload, ensuring the philosophy of person centred care is shared by the team and the patient/carer is central to the care planning process.

Supporting junior staff within their roles and aspirations is a vital component of this role .

Team work is the bedrock of a cohesive well functioning service .

 The ability to lead a team in a supportive manner is essential.
