This role is full-time and canbe worked flexibly across 4 days, a minimum of 30 hours is required, predominantlyoffice hours, with an expectation to work flexibility over a 7-day period ifneeded, with an option to work at home 1 day a week; dependent on need.
This role involves supportingfamilies holistically within their journey at Claire House childrens hospice.This role will be a main point of contact for families and professionals tobuild positive relationships, with a specific lead for social aspects of careand support.
Main duties of the job
This role will provide specialist resource in social care knowledge with a strong understanding of child and family social work principles, theories and practices.
To be responsible for following the journey ofchildren/young people and their families once they have been through thereferral process.
To be part of a care coordination team, overseeing thetotal caseload ensuring that all children receive the right service, at theright time, for the right amount of time. Acting as the main point of contactfor families and professionals, with a specific lead for social aspects of careand support.
To support and navigate systems and processes to maximisethe palliative care journey, encompassing a supportive and holistic approach tothe child/young person and their families with a life-limiting condition. Thepost holder is responsible as a registered, accountable and autonomous practitioner,to act in the best interests of the child/ young person and their family at alltimes. You will maximise the use of the multi-skilled team to plan, deliver,evaluate and maintain safe, compassionate and effective support.
As a RegisteredPractitioner, you will be required to demonstrate leadership qualities; beingopen, transparent and trustworthy. This is in accordance with Social WorkEngland registration and Claire House vision and values.

The post holder reports tothe Care Coordination Team Lead, and is accountable to the Director of ClinicalServicesClaire House is the placewhere you can make a real difference for children and families from acrossMerseyside, Cheshire and North Wales as well as a great place to develop yourcareer.
We recognise that working in aChildrens hospice is very different to that of other organisations andsometimes applicants are not sure which role best suits their experience. To support the assessment, planning, implementing, and evaluating care/support,such as behavioral assessment plans, based on current evidence-based andresearched best practice. Amendments will be negotiated with the child/youngperson and their parents/carers, documented and communicated to the othermembers of the team and external professionals
