They will be expected to assist the PCN team to deliver the Network Contract DES and also work with individual practices to assist with a number of enhanced services. The post holder will work across a number of our 11 GP Practices within the network
Main duties of the job
The pharmacy technician willsupport the pharmacy teamand assist a cluster of GPpractices. The purpose of this role is to lead improvements to maximise safe,cost effective best practice in prescribing to improve the quality of patientcare. The post holder will help patients to get the best from their medicinesthrough effective medicines reconciliation to agreed and approved protocols and improve the repeat prescribing processes. The will support and practices the team to reach targets within the PCN Network DES, Medicines Optimisation Service Level Agreement and QOF

The PCN has great access links and is located near junction 11 of the M6 motorway.

Main Duties and Responsibilities
Support the role of the ClinicalPharmacist at PCN level in the delivery The PCN Network DES
Medicines reconciliation postdischarge and on other transfers of care
Undertake medication use reviews
Day to day handling of prescriptionqueries within area of own competence
Use and maintain informationreporting systems and databases relevant to the position including incidentreporting, medicines optimisation, prescribing data, cost savings, and patientoutcomes
Participate in practice meetings,patient participation groups, and other meetings to improve engagement of therole of pharmacy technician and to promote issuesrelevant to prescribing and medicines optimisation
Liaise with other pharmacystakeholders Medicines Optimisation team, community pharmacy, and hospitalpharmacy teams.
Support the Pharmacy team in the delivery of the Service Level Agreement
Handle function specific information, which maybe sensitive, complex or confidential and appropriately recording, transferringand/or coordinating such information in accordance with the Data ProtectionAct; Caldecott Guidelines and the Confidentiality Code of Conduct
Be responsible for the organisation, planningand of own workload to meet set deadlines
Utilise Emis clinical systems to conductprescribing audits
Undertake, participate and share the outcomes ofclinical audits to facilitate improvement and changes in practice
Ensure that all major MHRA drug alerts, where appropriate and withinlevels of competence, are acted upon within practices over an appropriate timeframe and to advise on any medication changes that occur as a result
Regular monitoring of prescribingexpenditure at individual practice level and propose changes to ensurecontainment within identified resource
Undertake training and development, to ensurethe required knowledge and skills for practice based work
Carry out other duties which are appropriate to the skills andcompetencies of the post holder and grade of the post as the priorities of theservice change
Establish and maintain effectiveworking relationships with GP practices,ICB staff and other healthcare professionals/stakeholdersacross the interface
Provide clinical and professionalsupport to GPs and members of the primary healthcare team
Provide direct support and advice toGP practices and health professionals to promote and implement high quality prescribingto the benefit of patients
Promote cost-effective, safe,evidence based prescribing in accordance with national guidance and the localMedicines Optimisation Strategy
