Job summary

Due to promotions within the team an exciting opportunity has arisen for a Band 6 Diabetes Specialist Nurse who is passionate, highly motivated and who demonstrates the Vision of our Trust within our Integrated Diabetes team, on a 12 month secondment.

The post holder will work across both acute and community settings, ensuring the integration of care for patients whilst also expanding the knowledge and skills of the successful applicant.

You will be an enthusiastic and experienced nurse with a warm personality and great people skills. You will need to be adaptable to change and be able to influence new ways of working.

You will be willing to undertake relevant training and be able to demonstrate innovative practice.

You will be responsible for providing high quality evidence-based care for people with diabetes, their families, and carers.

To work as an integral member of the Diabetes team. Support the provision of specialist Diabetes nursing knowledge, advice and education to other nursing staff, the wider multi-professional team, allied health professionals and patient. Support the provision of education and training for nursing, allied health professionals and other health care professionals to promote insight into the specific needs of adults, including health promotion. The post holder will be part of a team who provides high quality Diabetes nursing care. Required to work flexibly to support the needs of the service, including 7 days working.

Main duties of the job

To deliver a high-quality patient centred system of care and education to enable people with diabetes manage their condition effectively.

To provide the highest standard of holistic care for patients through the utilisation of enhanced clinical skills and advice in the assessment and management of patients in collaboration with the multi-disciplinary team to improve and maintain the quality and safety of patient care.

To contribute to the delivery and development of the Diabetes service using effective evidence-based practice in line with agreed national standards for clinical practice.

To continuously improve clinical care standards in line with the clinical governance framework to ensure the highest quality of care is provided in safest manner.

To provide specialist advice and support to clinical staff within Salford Care Organisation on the management and care of patients within their own areaofspecialist practice and to facilitate the effective delivery of care to patients.

To always act as a resource and role model for the profession.

To provide education and training to clinical staff and internal /external agencies.

Support GP surgeries and Primary Care Networks in their management of people with diabetes to reduce variation in Primary Care through practice support and education.

Support the management of acute hospital admissions, including safe inpatient management and support/education for other health care professional colleagues within the inpatient setting.

This vacancy is open to those who have a current NMC PIN to practice as an Registered Nurse or Midwife in the UK, or those who are currently enrolled on a course which means they expect to receive their full NMC PIN within the next 3 months. Please note an NMC PRN PIN is not a full NMC PIN and would not fulfil the essential criteria for this role.

The NCA can no longer accept applications from International Nurses and Midwives who are yet to complete all competencies needed to receivea full NMC PIN to practice in the UK as we have paused our OSCE programme indefinitely. Person Specification

Education/Qualifications/Training

Essential

Registered Nurse on Part 1 of the NMC Educated to diploma/degree level or equivalent level of education. Evidence of further professional study

Desirable

Diabetes related qualification /evidence of study NMP V300

Continuous Professional Development

Essential

Able to demonstrate evidence of CPD. Able to demonstrate prior interest in diabetes via CPD evidence.

Desirable

Attendance at conferences, forums, study days or meetings related to diabetes management , Diabetes UK Annual Conference, local diabetes training. Attendance at modular courses related to diabetes management , insulin initiation, insulin pump therapy, carbohydrate counting.

Previous Experience

Essential

Evidence of post registration experience. Evidence of diabetes care experience. Experience of training or mentoring nurses and other health professionals Experience delivering patient/staff education

Desirable

Experience of working in the community Experience of designing audit/research projects Experience of liaising between primary and secondary care Experience of working within a multi-disciplinary team

Knowledge, Skills & Abilities

Essential

Up to date clinical or scientific knowledge of the management of diabetes mellitus Able to set priorities and organise own time. Demonstrable use of evidence-based nursing to develop practice. Demonstration of excellent interpersonal skills

Desirable

Able to manage own consultations. Participates in an active teaching role both formal and practical.
