- If the answer is yes to all of the above and you are highly motivated Band 3 health care support worker with an NVQ level 3, we are looking for you to join our Salford Community District Nursing team at Swinton.- The District Nursing Service provides care to patients and their families in their own homes and in a clinic setting. The teams are based at Lanceburn (inner city Salford), Broughton, Irlam, Swinton, Walkden and Eccles.**Main duties of the job**:
- You will work alongside a member of the team with full support of your Team Leader and other members of the team. Your caseload will be varied to your skills and training requirements. It will have a variety of patients with health and social care needs. This can vary from simple and complex patient care to looking after people at the end of their lives who wish to remain at home. You will also develop your competencies as part of the insulin delegation scheme providing diabetic care to the patients on the district nursing caseload. You will work closely with the MDT (Social Workers, GP’s and Allied Health Professionals) to ensure that patient care is appropriately coordinated.**Working for our organisation**:

- The ability to converse at ease with members of the public and provide advice in accurate spoken English is essential for the post
- Experience of working with members of the public or a desire to work in the Health Service
- Understanding of routine work procedures gained through a short induction period and on the job training
- Basic IT skills
