Job Summary

A great opportunity has come up to join Salford Integrated Diabetes Team.

Working in the community within an established service, this role will focus on patient education, aiming to evolve in innovative ways, while also aiding clinic workloads and involvement with diabetes and technology.

The post holder will:

  • Support and develop patient education programs for individuals with diabetes.
  • Work with patients with various complex needs in the diabetes field.
  • Deliver and audit structured patient education programs.
  • Manage Type 1 and Type 2 diabetes cases.
  • Provide primary care for diabetes patients.

Evaluate and treat a patient caseload while keeping autonomous records.

Assist patients through structured education programs and contribute to organizing relevant education initiatives and clinical services alongside the Highly Specialist Dietitian.

Main Job Duties

The post holder will effectively deliver, update, and enhance education programs for patients.

Conduct audits on current education programs and propose service improvements based on results.

Take clinical responsibility for a designated patient caseload and organize efficiently based on clinical priorities and time management.

Salfords Integrated diabetes service is a well-established team focused on patient care, empowerment, and excellence. The successful post holder will work with an experienced multidisciplinary team alongside Highly Specialist Dietitians and Diabetes Specialist nurses.
