Job summary

An exciting opportunity has arisen to join Salford Integrated Diabetes Team.

Working within the community setting within an established service, the post will have an emphasis on patient education, with the aim to develop in innovative ways, in addition to supporting clinic case loads and involvement with diabetes and technology.

The post holder will:

Support and develop a range of education programmes for patients with diabetes.

Work with patients who have a range of complex needs within the diabetes specialism.

Delivery and auditing of structured patient education programmes Management of Type 1 and Type 2 diabetes Primary care of patients with diabetes

Assess and treat a caseload of patients and maintain own records as an autonomous practitioner.

Support patients through several structured education programmes.

Contribute to the co-ordination of relevant education programmes and related clinical services alongside the Highly Specialist Dietitian.

Main duties of the job

The post holder will effectively deliver and, where appropriate, further update and develop education programmes for patients.

To audit current education programmes and, based on results, suggest and help implement service improvements.

To accept clinical responsibility for a designated caseload of patients, and to organise this efficiently and effectively with regard to clinical priorities and use of time.

Salfords Integrated diabetes service is an established team with an emphasis on patient care, empowerment and excellence. The successful post holder will be supported by an experienced and effective multi disciplinary team, working closely with Highly Specialist Dietitians and Diabetes Specialist nurses.
