Job summary

This new and innovative post has been developed through Macmillan to improve personalised care and support offered to Non-Surgical Oncology (NSO) patients attending Weston Park Outreach services.

This is an exciting time as we make changes to the way we deliver treatment to enhance the care for our patients who are living with and beyond cancer through direct on-site support, which will enhance patient experience, promote continuity and strengthen communication.

The main duties of the role is to support patients with their non-clinical needs and support patients receiving out-patient care away from their usual, local hospital. Acting as a main point of contact for patients, carers and wider healthcare teams ensuring the joining up of services, coordination, information and communication between all creates a seamless experience of care.

The post holder will need excellent communication skills and the ability to work as part of a multi-disciplinary team. As this is a pilot good IT skills are necessary to ensure accurate collection of data and patient experience.

The post holder will support people to navigate the complex health and social care system, providing input to patients from each of the respective 4 Trusts outside the tertiary centre (Barnsley, Rotherham, Doncaster, and Chesterfield) and become a regular point of contact for patients attending, enabling greater continuity by acting as a conduit between the patient and their site-specific CNS team.

Main duties of the job

To assist the NSO CNS within the daily management and support of people on a NSO pathway, from diagnosis transitioning through to supportive and palliative care. Acting as a source of support and information to patients and their families/carers, further aiming to identify any potential delays within the patient pathways, working to support and minimise the impact of these problems to the patient.

This important role is to help support and navigate patients, their families and carers through the Cancer pathway ensuring that the patient receives the very best cancer pathway experience with continual communication and support.

The role requires someone with exceptional organisational skills who can demonstrate great communication and interpersonal skills. A caring and sensitive attitude is essential as you will be dealing with patients, their families and carers.

The post holder will work closely with clinicians (Consultants and Cancer Clinical Nurse Specialists), Managers and Booking Staff in order to identify and resolve delays in the pathway. They will be responsible for closely managing the patient tracking list (PTL), escalating operational issues and maximising available capacity by providing feedback to both clinical and non-clinical staff.

This vacancy is advertised on a fixed term contract basis for a period of 12 months due to the post being externally funded. The fixed term contract status of the post holder will be reviewed in accordance with the Trust's legal obligations regarding fixed term contracts taking into consideration the ongoing source of the external funding and the requirement for the post to be maintained.

Person Specification

Qualifications

Essential

4 GCSE's Grade C or above including Maths and English or equivalent qualifications or experience. NVQ or QCF level 3 or working towards in Health or Health and Social Care Evidence of foundation degree level study Coaching or teaching courses and/or qualification or equivalent relevant experience Computer literacy with good understanding of computer packages word/excel, and knowledge of confidentiality, information governance and data security

Desirable

Care Certificate completion or willing to undertake self-assessment Basic counselling qualification

Experience

Relevant Health or social care experience at Band 3 or equivalent, or transferable knowledge & experience of working with the public Understanding of key referral pathways to Allied Health Professionals to support plans of care Experience in managing small projects Involvement with Data collection systems Experience of dealing with challenging situations Experience of working within a multidisciplinary team, or equivalent Experience of working in a busy environment that requires flexibility
