Job summary

The Advanced ClinicalPractitioner will undertake clinical assessment including history taking andphysical examination, form a diagnosis and provide a treatment plan, which mayinclude prescribing. The Advanced Clinical Practitioner will demonstrate safe,clinical decision making and expert care. The Advanced Clinical Practitionerwill be expected to treat all ages from children to the elderly, withundifferentiated presenting complaints, including mental health, paediatricillness and palliative care. Top up training can be provided in any areas ofidentified development.

The Advanced ClinicalPractitioner will work collaboratively with all membersof the team. This may include telephone consultations, face to faceconsultations, or mobile/home visits. At times you may be operating as a singleclinician. Considering all four pillars of advanced practice, you are expectedto utilise your clinical leadership skills and supporting educationaldevelopment of junior staff.

Main duties of the job

-Will work autonomously within a health care teamconsisting of Team Leaders, Advanced Clinical Practitioners, Duty Doctors,Call Handlers & Receptionists.

-The composition of this team will vary from shift toshift & ACp's must be able to communicateeffectively & professionally with all staff members.

- -As part of a national service, you will deal with callsof a very diverse nature covering all clinical aspects & issues, from aninfinite variety of callers, whose cultural, social & emotional diversitiesrequire to be acknowledged & taken into consideration during theconsultation

-You will be required to use a multi skilled approach.Whilst using the available technology, the ACP will use effective listening & communication skills in order to make clinical decisions, create records & refer to clinical reference literature during the consultation period.

--Must be able to retrieve & use information regardinghealthcare provision in the patients locality, allowing appropriate care to beaccessed where necessary.

--Assess, diagnose, plan, implement and evaluatetreatment/interventions & care for patients presenting with anundifferentiated diagnosis

-Diagnose & manage both acute and chronic conditions,integrating both drug- & non-drug-based treatment methods into a managementplan

--To provide face-to-face & telephone clinicalassessment/treatment and management plan to patients.

-Ensure that all treatments & care provided are based on best practice/evidence-based care

Our aim at Greenwich PCN Alliance Limited is support the improvement of primary care across Greenwich by providing support to Primary Care Networks (PCN) across Greenwich and recruiting Additional Roles via the the Additional Roles Reimbursement Scheme (ARRS) .

Primary Responsibilities:

Clinical:

Assess, diagnose, plan, implement and evaluatetreatment/interventions and care for patients presenting with anundifferentiated diagnosis.

Diagnose and manage both acute and chronic conditions,integrating both drug- and non-drug-based treatment methods into a managementplan.

Prescribe and review medication for therapeuticeffectiveness, appropriate to patient needs and in accordance withevidence-based practice and national and practice protocols, and within scopeof practice.

Work with patients in order to support compliance withand adherence to prescribed treatments.

Provide information and advice on prescribed orover-the-counter medication on medication regimens, side-effects andinteractions.

Prioritise health problems and intervene appropriatelyto assist the patient in complex, urgent or emergency situations, includinginitiation of effective emergency care.

Assess, identify and refer patients presenting withmental health needs in accordance with the NSF for Mental Health.

To provide face-to-face and telephone clinicalassessment/treatment and management plan to patients.

Help in the development and evaluation of clinicalguidelines, and competency tools to support practitioners in the expansion oftheir roles in the delivery of optimal care.

Undertake consultations withpatients taking a comprehensive patient history, carry out physical examinationand use your expert knowledge and clinical judgement to identify the potentialdiagnosis.

Refer patients forinvestigations when necessary/appropriate.

Make a final diagnosis,decide on and carry out treatment, including the prescribing of medicine orreferring to primary or secondary care specialist services within local agreedpathways and protocols as appropriate.

Ensure that all treatmentsand care provided are based on best practice/evidence-based care.

Asses and evaluate, with thepatient, the effectiveness of the treatment and care provided and make changesas needed.

Use independent prescribingskills for benefits of patient care.

Providesupport/leadership/training/education.

Identify and takeappropriate actions to address risk to the patient, this will include:Identification of emergencycare needs requiring dispatch of a 999 ambulance and safeguarding children andprotection of vulnerable adult issues which require referral on to otheragencies in line with local policies.

Work independently and atthe same time contribute to the work of the whole team to ensure the deliveryof a consistently high level of service, this will include; taking responsibility andbeing accountable for managing own workload, risk assessment and management,acting within professional codes and appropriately to the clinical team leaderfor advice and supporting. Acting as a clinicalresource for non-clinical staff who may require advice and support.

Communication:

Communicate effectively with patients and carers,recognising the need for alternative methods of communication to overcomedifferent levels of understanding, cultural background and preferred ways ofcommunicating.

Anticipate barriers to communication and take action toimprove communication.

To be aware of responsibilities under the DataProtection Act.

Attend regular team meetings.

Provide a professional,courteous and efficient service consistent with organisational standards forpatients seeking assistance from the service, maintaining a professional andreassuring manner at all times.

Utilise advanced listening,probing and facilitative skills across a diverse range of cases, some of whichmay be highly challenging due to emotive circumstances, communicationdifficulties and patient anxiety.

Utilise complexcommunication skills to negotiate and provide support to patients who may benon-compliant with the recommended outcomes.

Utilise interpreterservices, communication aids and other resources, to address barriers tocommunication and understanding.

Effectively use a range ofcommunication methods ( telephone, email, fax, letter and report writing)to facilitate communication at all levels within and external to theorganisation.

Audit:

For clinical governance and training purposes, regular monitoring is requirement of this job.

Education, Training and Development:

Contribute to theachievement of service standards and performance requirements by participatingin regular quality monitoring and individual performance reviews.

Attend and contribute tomeetings, workshops and other groups in order to support the on-goingdevelopment of the service

Participate in audit,customer satisfaction programmes, research and other quality improvementactivities.

Participate in the investigationof incidents, complaints and other issues as required.

Manage and maintain clinical records as required.

Maintain active NMC registration.

Tocarry out other duties commensurate with the grade as directed,

Experience of working as a practioner in a GP surgery, primary care environment, emergency department, urgent care centre, MAU/EAU or other first contact setting, evidence of excellent assessment skills. Experience of telephone triage/assessment environment. Experience of working autonomously diagnosing and treating patients. Experience in mental health assessment. Experience in paediatric assessment. Knowledge of equal opportunities and its significance in health care. Awareness of mental health, child protection and safeguarding vulnerable adults issues.

Desirable
