Detailed job description and main responsibilities

Applicants must have a full valid driving licence and have access to a car for work. If necessary, adjustments can be considered in line with the Equality Act 2010. 

The service operates 7 days per week, 8am to 6.30pm. Full or part time (minimum 22.5 hours) roles are available. Annualised hours contracts will be considered.

Essential criteria

Registered General Nurse (adult). Educated to degree level or equivalent

Knowledge and Experience

Essential criteria

Knowledge around long term conditions and their management. Evidence of ongoing professional development.

Desirable criteria

Previous Community Services experience.

Technical expertise/competence

Essential criteria

Demonstrates an understanding of local and national issues influencing the delivery and development of health care. Demonstrates a knowledge and understanding of evidence based care. Demonstrates an understanding of the Clinical Governance requirements. All new starters have a 6 month probationary period. This, together with the induction process, aims to create a positive supportive working environment allowing new employees to learn key elements of their role over a reasonable timescale Candidates not currently employed by the Trust who attend an interview for non-qualified Band 1-5 posts are required to undertake numeracy and literacy assessments. Appointment to this post is subject to the trust receiving satisfactory references covering 3 years of employment or study. Please ask your referees to respond promptly to reference requests. Employees are expected to undertake mandatory and statutory training related to their role.
