JobSummary:

FamilySolutions (Edge of Care) is a service designed for young peoplebetween the ages of 11 and 17 years old and their families wherethe young person is considered by the local authority to be incrisis and consequently on the cusp of a reactive careepisode.

A team ofdedicated and sed practitioners work alongside the allocated socialworker and the family with the aim of identifying workablesolutions to help improve the relationships they share with eachother.

KeyDuties/Accountabilities:

The FamilySolutions Service works with the whole family utilising strengthsbased and solution focused odology to support them in developingand implementing sustainable long term desired change. Interventionis intensive and time limited and as a consequence caseloads arecomparatively small and offer the opportunity for direct andfrequent engagement at the heart offamilies.

The Edge ofCare Family Solutions Practitioner will work as the leadprofessional working closely with families and partners to ensurefamily members needs are met using the whole family andstrengthsbasedpractice.

To ess theneeds and risks for children and identify actions appropriate tomeet need and address risk in collaboration with referring socialworkers.

They will workalongside allocated SocialWorkers

EssentialExperienceRequired:

Experience ofacting as a lead professional for families with multiple or complexneeds and working directly with families to deliver positiveoutcomes forthem.

Experiencecollaborating with social workers education police and voluntarysector colleagues and other professionals as the lead professionalin multiagencysetting.

EssentialQualificationsRequired:

A recognisedqualification in work with children or young people. Recogniseddisciplines include social work teaching youth work health workpolice and probationwork.

Experience of acting as a lead professional for families withmultiple or complex needs and working directly with families todeliver positive outcomes for them.
