This role is applicable to an experienced Nurse with over 12 month's qualified experience, you must have completed preceptorship. Please note this is not applicable to a newly qualified nurse without 12 months experience.

An exciting development opportunity has arisen for a Band 5 Community Practitioner to work within the speciality field of a Community Treatment Team based at Palmer Hospital, covering South Tyneside area.

Due to the area of expertise of the service, we have developed a bespoke programme to support those without experience in this specialist field to learn and develop their skills over a 12-month period, where you will receive mentoring and training on the job. At the end of the 12 months, you will automatically progress to a Band 6 Community Practitioner, if the competencies have been achieved.

South Tyneside Community Treatment Team is a multidisciplinary service which provides assessment and treatment for people experiencing moderate to severe mental health problems. your role will incorporate working closely with partner agencies/services in delivering effective care and treatment.

In collaboration with the Service User and their families, the post holder will facilitate formulation which then informs a Recovery and Wellbeing focussed Care Plan, ensuring regular reviews to monitor progress and adapt the Care Plan as necessary.

Main duties of the job

They will work as a member of the Multi-disciplinary Team ensuring that high quality individualised care is delivered which ensures safety and a commitment to promoting resilience, recovery and wellbeing and maximising independence.

The successful candidate will be responsible for the assessment, management and care coordination of service users who have a variety of complex needs, diverse presentations, and a range of mental health conditions.

Assessment of clients presenting with significant mental health needs.

Developing collaborative care plans with clients and their carers

Delivering evidence based intervention to support with recovery

Identifying safeguarding concerns and responding accordingly

Assessment and management of risks

Working within an MDT

Completion of electronic records as per policy and professional body

Working with other agencies to provide appropriate care and treatment

Engaging in supervision processes

Continuing professional development.
