A great opportunity has become available in our friendly and supportive team, Winchester Rural South for a Senior Clinical Community Nurse. We are looking for an enthusiastic individual to fill our Band 6 role, to work in our mixed professional team in both patient homes and clinic settings.

Service aims include providing a local point of access, proactive case management and adoption of a co-produced approach with patients and service users.

The successful post holder will be required to provide expert clinical management for patients with complex care needs, management, support and leadership for the community nursing team supported by the Community Team Matron.

You will also be responsible for the recruitment, induction, clinical supervision and day to day/operational management of the Community Nursing Team and ensuring an effective contribution to the implementation of SHFT clinical, governance and other strategies.

The Winchester Rural South team provides cover to a large rural community within the Winchester district between Twyford and Wickham between the hours of 0800 am and 2000 7 days a week.

Main duties of the job

The primary role of the Community Team Lead is to provide strong holistic leadership to the team and support the Community Team Matron (Band 7). To include day to day leadership responsibilities, development and creative leadership, responsive leadership, enabling the team to achieve a 'well led' culture.

The Community Team Lead in conjunction with a wide range of clinical colleagues and specifically, GP's, Practice Nurses and Social Care professionals, will lead and facilitate a patient or client focused, co-ordinated case management approach across primary and secondary care for people who are most vulnerable to, and at high risk of repeat admissions to hospital.

The Community Team Lead will participate in and influence efforts across health and social services to shape multi-disciplinary pathways designed to support patient choice, improve quality of life, promote self-management and assure early intervention through the proactive provision of care in or as close to the patient's own home as possible.

The Community Team Lead will work across the caseload and the single point of access or (equivalent), using their clinical skills to identify the needs of patients and the correct services to liaise with.

The Community Team Lead will provide an expertise in their own discipline to the wider team.
