Job summary

An exciting opportunity has arisen to join the Renal service team at St Helier Hospital as an MDT Service Coordinator (non-clinical).We are looking for an outstanding team player, an individual with a keen eye for detail and someone who can multi-task and independently prioritise their workload. The individual must be highly motivated and successful candidate will coordinate and run the Cardio - Renal - Diabetes MDT meetings, as well as undertake all associated administrative tasks, including tracking patients on their pathway.

The successful candidate will also coordinate and support the SWL ICB funded Supportive Care clinics and MDT for the Renal division.

Main duties of the job

Ensure effective patient management plans for all patients referred into the Cardio-Renal-Diabetes MDT and SWL ICB funded Supportive Care service either via eRS, GP or consultants, for patients cases to be discussed with all relevant notes, imaging and treatment information available within 2 weeks of referral.

To work as part of the Cardio - Renal - Diabetes multi-disciplinary team, acting as a liaison between consultants, nursing teams, community and primary care professionals, South West London ICB, admin teams, outpatient team, scheduling team, therapies team and theatres.

To ensure a smooth patient pathway from MDT, to outpatient, inpatient and community follow ups after Consultant / Specialist Nurse and / or MDT Review and management plan has been completed.

Attend the regular MDT meetings and record outcomes.

Recording outcomes on the spreadsheet and actioning eRS

Distributing actions to all relevant teams and clinicians individually.

To communicate with patients about their intended management plan.

To check appropriate investigations for all patients.

To upload all clinical information to individual patient records.

The post holder will work on their own initiative and without direct supervision to provide a proactive comprehensive, high quality and effective senior administrative support to the service and clinical lead.
