**Responsibilities**:

- Commence discharge planning discussions with MDT from day one of admission/ as soon as possible after admission.
- To promote that the Estimated Date of Discharge is set meaningfully and realistically in the first instance, reviewed daily and accurately recorded
- Maintaining records for monitoring and audit purposes ensuring the data produced is accurate and reflective.
- Identify next of kin and / or significant family member to support patient’s discharge plans.
- Identify need for completion of relevant referral or assessment paperwork and liaise with appropriate team members to aid timely submission, e.g. onward care referrals.
- Take ownership of the Criteria to Reside notification and reporting process for the ward area on Apex
- Ensure discharge letters/ steps of the managing discharge policy are undertaken in the correct and timely manner.
- Provide patients and next of kin with discharge planning information including relevant part of “Managing Complex Discharge Policy” for the Trust.
- To assist patients in moving to the Discharge Lounge
- Act as an advocate to patients when appropriate to ensure their discharge is safe and effective.
- Ensure plans for transferring patients to community services are progressed with the patient, next of kin and MDT to facilitate discharge from hospital once deemed medically stable
- Act proactively to provide a reduction in length of stay and delayed discharges
- Liaise with care agencies and care homes to establish dates for assessments and agree transfer dates; work alongside social services, CCGs and trusted assessors to ensure return to placement and restarts of care packages are completed following policy
- To escalate any delay within the discharge planning process to appropriate team leaders, clinical leaders and service managers. This may include MDT assessments, diagnostics, family, care home delays etc.
- Liaise with the bed managers to ensure they are aware of all medically fit patients or when discharges are at risk
- Participate in the weekday, weekend and bank holiday rotas as eRostered
- Ensuring confidentiality at all times
- Promote person-centred approach to transfers of care by ensuring detailed work up of patients’ needs and desired outcome.

- maintain a constant awareness of health, welfare and safety issues affecting colleagues, patients, visitors and themselves, reporting any accidents or fault in line with trust policy,
- fully participate in health and safety training,
- participate in personal training, development, appraisal, and attend all relevant training courses as required.
