Job summary

Inpatient Rehabilitation Support Worker

Full time, permanent position, up to hours per week, Band 2

This role attracts paid enhancements for out of hours, bank holiday, weekend and night working.

We have an exciting opportunity for an enthusiastic support worker to join our established inpatient team. You will support both nursing and therapy teams to facilitate rehabilitation in an inpatient environment. Your role will be patient facing and very much hands on, supporting patients with activities of daily living.

Our aim is to promote recovery and well-being, whilst empowering patients to self-manage and maximise their independence using meaningful activity. We take a very holistic approach to rehabilitation working on the ethos that all patient interactions have the potential to be a rehabilitation opportunity.

The unit also supports inpatient Specialist Palliative Care patients.

Main duties of the job

You will be part of a team of support workers trained in a range of skills. Your role will be very physical, providing hands on care including assisting with all ADLS as needed. You will work alongside both nursing and therapy teams, delivering care so that they can support and respond to the patients needs. This includes all aspects of personal care, with an ethos of rehabilitation to support patients to gain as much independence as possible. You will also support end of life patients and their families over the last few weeks/days of life.

The Unit presently comprises of 20 intermediate care beds delivering rehabilitation and 6 Specialist Palliative Care (SPC) beds in a purpose designed setting. For our intermediate care patients our aims are to reduce acute hospital admission and facilitate early discharge by utilising the skills of the multidisciplinary team of nurses and therapists. For the SPC patients we aim to provide holistic symptom management.

Job responsibilities

Contribute to, support and promote ECCHs, strategic direction, values and culture in relation toproactive and reactive services.

Communicate effectively with patients, relatives andcarers, respecting patient confidentiality withprivacyand respect for diverse cultural backgrounds and requirements

Support and deliver aspects of activities of daily living.To include management of dietary and fluidintake,toe nail care, management of continence, assisting with toileting programmes,including use ofsheaths, catheters and continence pads.

Undertake and record urinalysis, observations such aspulse, respiration rate, blood pressure.

Reportingany changes to a Health Care Professional

Participate in one to one meetings/clinical supervisionwithin team as required.

Participate in the learning environment.

Contribute to the clinical governance agenda throughparticipation in clinical audit when required.

Contribute to an environment conducive to effectiveworking, respecting and supporting thedeliveryof high-quality clinical services.

Contribute to change and innovation within the team.

Work as part of the multidisciplinary to deliver delegatedproactive including reablement and carrying out therapy exercises

Support therapy staff with access/discharge home visits

Support therapy staff to deliver various activities to aidfunctional ability group exercises, games and puzzles to improve manualdexterity and memory

Work in partnership with senior members of the team andensure the care provided is of high quality

Work with patients towards self-care and independence.

Work to support families and carers. Be aware of theeffects of illness on the family

Enableindividuals and their families to express their feelings and preferences andtake appropriateaction.

Work as part of the multidisciplinary team to deliversupportive care to patients approaching end oflife.

To promote effective communication between patients,families and carers to include participationin alldecision making related to their care

Follow an agreed care plan offering a range of clinicalactivities to ensure best practice is delivered.

Undertake personal care activities of daily living withpatients

Undertake skin assessments, continence and nutrition andreport back to senior colleagues

Take,Record and Report (if outside of normal parameters) Blood Pressure, Respiration,Oxygen Saturation and Pulse rate as may be necessary using NEWS2. (NationalEarly Warning System 2 Observation chart)

Undertakeurinalysis, including collecting urine sample as directed; reporting anyconcerns regarding urination.

Participate in Health Education/training for patients,families and carers

Engage in personal development; maintain competence,knowledge and skills commensurate with role

Be wholly accountable for practice taking every reasonableopportunity to sustain and improveknowledgeand competence

Provide information as may be required to complete outcomemeasures on admission anddischarge.

Maintainlegible, accurate and contemporaneous patient records in accordance with ECCHPolicy.

Report any changes in patients condition to a registeredHealth Care Professional

Take responsibility for the safe use of clinical equipment
