Job summary

Working as part of the provider collaborative thepost holder will provide case manager support for a designated caseload ofservice users within inpatient units across the South West region and out ofarea. This will include robust oversight of access assessments, ensuring thateach service user is placed in the least restrictive setting with theappropriate level of supportive interventions as close to home as possible. Asa Case Manager the post holder will work with the pathway managers, ensuringpatients have goal orientated treatment and follow appropriate care pathway todischarge. As a Case Manager the post holder will be supported bysenior colleagues, with a reduced caseload and no requirement to be on call.Case managers work with the pathway managers, ensuring patients have goalorientated treatment and follow appropriate care pathway to discharge. The rolewill support the services in driving transformation as well as value for moneyin planning, commissioning and service delivery. The role is designed to use acombination of clinical expertise and technical skills to map future serviceuse, identifying and addressing pathway barriers that prevent the repatriationof service users out of area and identifying and addressing challenges thatprevent timely discharge or transfer of service users. The post has a criticalfunction in assessing clinical quality issues and identifying any improvementsrequired.

Main duties of the job

As a Band 7 Case Manager, the postholder will work as part of a dynamic team to deliver a service that ensuressafe and effective care pathways are in place for service users of specialisedmental health care settings within the NHS and the Independent sector. The postholder will be required to support clinical staff and managers across the SouthWest Provider Collaborative footprint. Caseloads vary but will include workingwith patients within our Adult Secure Services; Adult Eating Disorders, CAMHS Tier 4 and Perinatal.

This role will carry a smallercaseload and be offered increased supervision than the Band 8a Case Managers.Working as part of a busy team the role includes assisting in access assessmentquality reviews, ensuring referrals and admissions are managed and appropriate,and that the quality of patient safety, care and treatment is paramount. Thesuccessful applicant will be able to demonstrate high levels of skill workingwith complex individuals, their families and carers within a variety of caresettings.

Registered Mental Health / Learning Disability Nurse or other registered health or social care professional

Must have an understanding of the background to and aims of current healthcare policy and appreciate the implications of this on engagement Must have an appreciation of the relationship between the Department of Health and individual provider and commissioning organisations.

Knowledge of legal issues pertaining to the specialist service, including MHA 1983, after care under supervision, sex offender legislation, probation orders and life licence, Ministry of Justice procedures and child protection legislation Knowledge and experience of risk assessment and risk management. Development and maintenance of therapeutic alliance with service users and their families /carers. High-level management skills, able to perform to high level in clinical setting, holding services to account and ensuring effective use of resources. Case management skills, focussing on the needs of the individual and the context of their pathway in relation to all clinical and legal processes Ability to analyse complex facts and situations and develop a range of options.
