Primary Care Networks (PCNs) form a key building block of the NHSlong-term plan. Bringing general practices together to work at scale has been apolicy priority for some years for a range of reasons, including improving theability of practices to recruit and retain staff to manage financial andestates pressures; to provide a wider range of services to patients and to moreeasily integrate with the wider health and care system
Following the release of ARRS GP funding we are looking for newlyqualified (within 2 years) to Join Great Sutton Medical Centre on afixed term salaried contract until 31 March 2025. We are looking for 5 sessions
Our clinical system is EMIS Web and we have astructured administrative team to help with letters, results, medicationmanagement, etc.
Working as part of our multidisciplinary team, you will have theopportunity to support and care for a diverse patient group
Main duties of the job
The successful candidate will undertake a variety of duties including surgery consultations, telephoneconsultations and queries, visiting patients at home, checking and signingrepeat prescriptions and dealing with queries, paperwork, reports andcorrespondence in a timely fashion
Making professional, autonomousdecisions in relation to presenting medical problems, whether self-referred orreferred from other healthcare workers within the organisation
Assessing the healthcare needsof patients with undifferentiated and undiagnosed problems;
Screening patients for diseaserisk factors and early signs of illness;
Developing care plans for healthin consultation with patients and in line with practice disease managementprotocols;
Recording clear consultationnotes to agreed standards;
Collecting data for auditpurposes and complying with QOF requirements;
Compiling and issuingcomputer-generated acute and repeat prescriptions;
All general duties andresponsibilities associated with a GP working within primary care.

We have a number of excellent additional roles (ARRS) within our PCNincluding social prescriber link workers, clinical pharmacists, pharmacytechnicians, health and wellbeing coaches, dieticians,care co-ordinator, occupational therapist and physiotherapist which offeradditional patient care and allow the Doctors to focus on patient care andsignpost to the most relevant colleague or team.
The practice has secured a rating of GOOD by theCare Quality Commission.
