Job summary

We are looking for a highly motivated, knowledgeable and enthusiastic qualified Advanced Clinical Practitioner (ACP) to join our Cardiology ACP team.

The team was formed over 10 years ago and currently consists of 13 qualified ACP's and 2 trainee ACP's who are either Nurses or Pharmacists by background.

The successful candidate would join the ACP rota to enable us to expand our cover to Patience 1 at Nottingham City Hospital which is our heart failure ward. We can be flexible with the candidate's job plan. If they have a particular interest in heart failure, they could work predominantly on Patience 1. If they have an interest in general Cardiology, they could join the team on a rotational basis providing cover for Chest Pain and ACS clinics at Nottingham City Hospital (NCH), Cardiology wards at NCH and an in-reach service to the Emergency Department and Acute Medical wards at Queens Medical centre.

There will be opportunities to join the ACP team in shared ACP pillar and CPD activities.

Main duties of the job

If you are an ACP in another clinical area with relevant qualifications but are looking to change your area of practice, we will be able to offer a supervised practice/training role to enable you to develop your skills, competencies and a portfolio within a new speciality. If appointed, we will support you with a period of supervision and assessment tailored specifically to individual needs and requirements.

Your clinical development will be supported and includes access to a well-developed post graduate education plan. As an ACP at NUH, your development would follow a Trust-wide five year ACP plan encompassing the four pillars of advanced practice. During this period, it is expected that the individual will be actively involved in developing their speciality within local, regional and national forums and networks.

Job description

Job responsibilities

You must have completed an MSc in Advanced Clinical Practice, which includes Non-Medical Prescribing and be able to present a portfolio demonstrating evidence of your ACP clinical competence.

You will undergo a period of supervision and assessment to ensure you are embedded correctly into our workforce and to develop skills/portfolio within Cardiology. You will have a dedicated Consultant Supervisor that has dedicated time within their job plan.

Thepost holder will rotate to the Cardiology clinics (NCH), Cardiology wards (NCH) and Cardiology in-reach team (QMC).

When working in the Cardiology in-reach team, you will be required to:

Accept referrals from the Emergency Department and the Acute Medical wards/Admission units for patients with suspected Cardiology problems who require specialist input from the Cardiology team.

Formulate appropriate management plans for patients with suspected angina, acute coronary syndrome, heart failure, cardiac arrhythmias and other cardiac conditions.

When working in the Chest Pain clinics, you will be required to:

Accept referrals from General Practitioners (GP), Emergency Medicine and other NUH specialities for patients with suspected angina who require specialist input.

Manage a caseload of new outpatients with suspected angina. This will include interpretation of investigations to deliver a clear diagnosis to the patient and the provision of a clinic summary letter for the GP outlining the ongoing management plan.

When working in the ACS clinic, you will be required to:

Accept referrals for patients who require out-patient follow up after being admitted with an ACS or post PCI.

Manage a caseload of outpatients who have had a recent ACS. This will include management of any investigative results and the provision of a clinic summary letter for the GP outlining the ongoing management plan.

When working on the Cardiology wards, you will be required to:

Clerk and review patients on Cardiology wards, assisting in reaching a diagnosis, ordering diagnostic tests, chasing and interpreting results.

Ensure patients are fully prepped for investigations and facilitate prompt discharge.

Complete a comprehensive discharge summary.

Along with clinical work across the department, the individual will have weekly rostered time to undertake their own continued professional development alongside service development work. They will be given the opportunity to follow personal interests and pursuits whether that is in education, teaching, research or management.
