The Community Therapy team work with adults who have a wide range of disabilities which maybe acute or long term, but often require flexible and creative case management, clinical tenacity and professional accountability in order to deliver excellent holistic patient care. Our work is consistently varied and stimulating covering admission prevention and supporting discharge, with a dedicated focus on rehabilitation in a range of Community settings, utilising medical, social and public health approaches. The Community Therapy Team works within the broader Health and Social Care team alongside colleagues from Community Nursing and Adult Social Care.
We are a positive, progressive and proactive team and adapt to the changing needs of the patients we work with and the service demands. If you are flexible and creative in your approach to work, enjoy working in a multi-disciplinary team, have an interest in developing yourself and others, this could the opportunity for you. In support of Community based staff, our cluster has a fleet of shared pool cars that Clinicians are able to book to aid Clinical visits.
