Livewell Southwest are currently lookingfor Social Workers within it Discharge to Assess Pathway.

Social workers primarily work with peoplein complex situations. Complex is defined as - unpredictable or ambiguoussituations where the person may be resistant, coerced or a significant risk toself and others. These situations are often time-consuming owing tothe multiple factors that have led the person to becoming socially excluded.Decisions require proportionality and analysis of many different views andfactors, such as psychological, biological, legal, environmental andeconomical.

Main duties of the job

The post holder is requiredto discharge statutory social care functions on behalf of the organisation andto uphold the standards outlined in the Professional Capabilities Framework forSocial Work. As such, they are required to undertake complex and proportionateassessments that are evidence-based. Examples include Mental Capacityassessments and Care Act assessments of people who have a variety of needs thatmay create a high level of risk to their wellbeing. The post holder is alsorequired to arrange and review the delivery of social care support.

Wherever possible the postholder is required to support people to take risks in complex situations sothat their outcomes and wishes are promoted. The post holder is also requiredto undertake safeguarding investigations and consider decisions made in apersons best interest that may deprive them of their liberty.

All Livewell Southwest staff are expected to able andwilling to work across a 7 day service.

Job responsibilities

Discharge to Assess (DTA) is a dynamic and multi-skilled team consisting of nurses, social workers, occupational therapists, physiotherapists, community care workers and support workers. Our purpose is to support a safe adn timely hospital dicharge of peopel who are medically fit, and no longerneed hospital care, to a place more suitable to teh persosn's needs. Assessments and care provision can then be tailored to support peopel to regain strenghts and skills so that they can live as indepndently as possible.

The DTA model aims to support better outcomes of peopel leaving hospital.

Person Specification

Qualifications

Essential

A qualification in Social Work, such as postgraduate diploma, bachelor degree or masters level.

Desirable

Any postgraduate qualifications relevant to social work, such as Graduate Certificate in Practice Education or Approved Mental Health Professional

Experience

Essential

Proven relevant experience of working in the health and social care sector. Knowledge of social work theories and models, the strengths and limitations and how they can be applied to practice. Knowledge of the legal framework relevant to social work, including the Care Act (2014), Mental Capacity Act (2005), Mental Health Act (1983) and subsequently in 2007; and The Human Rights Act (1998). Knowledge of the Professional Capability Framework and the standards set by the social work regulatory body, and maintaining professional registration.

Desirable

Experience of working with other professionals in an integrated service.

Knowledge and skills

Essential

Have excellent interpersonal and communication skills Maintain positive working relationships and communicate clearly with team colleagues, line manager, other professionals, people who use our service and their families/carers which may include highly sensitive issues. Awareness of current social care research trends and to take part in development activities, such as Safeguarding Adult Reviews. Participate in and undertake Risk Management in complex and challenging situations. Be able to work effectively in an integrated and Multi-disciplinary team environment to support better outcomes for service users Supervise and support Band 5 colleagues as and when required to ensure their practice is safe and professional. Have a good understanding of the various computer programmes essential for the completion of daily work, such as Eclipse , Salus, CF6 Microsoft office. Undertake holistic assessments, implement and review care and support plans to meet identified needs , Consider best value option when planning for peoples care and support, including awareness of 3rd sector provision and assistive technology Undertake statutory duties and assessment including Risk Management, Mental Capacity, Best Interest , Carers, CHC in accordance with relevant legislation and guidance Take flexible and adaptable approach to organisational change.

Desirable

Discharge to Assess cover a 7 day working week and therefore there is the expectation that the appointable social worker be flexible and prepared to work some weekends on a rota basis
