Job overview

We are recruiting a consultant geriatrician to join our exciting and rapidly expanding Frailty Team delivering integrated holistic care spanning both acute and community settings. The post holder’s primary duty will be to develop pathways and enhance the reactive frailty services, UCR, virtual wards, ED front door and Mary Seacole Unit. In addition, the post holder will have opportunities to support the integrated frailty service and be involved in providing reactive and proactive care across the acute and community areas. There will also be responsibilities in supervising and teaching trainees and medical students, as well as nursing and allied health professionals of the MDT. The post holder will drive forward developments, improving the quality of care delivered to patients with a focus on maximising peoples’ independence and supporting their return to their usual place of residence wherever possible. You will receive peer support from fellow Consultants working across the frailty services in Epsom General Hospital and Surrey Downs Health and Care and be fully supported by an integrated management team helping you to driveforward change.

We request that successful applicants   must have relevant UK and NHS experience in Geriatric and General Internal Medicine with full GMC registration and already have CCT, or be within 6 months of obtaining their CCT.

Main duties of the job

Consultant geriatrician as part of the Frailty Team covering the following clinical areas:

Community reactive services (UCR and VW) – providing clinical advice and leadership to the MDT. This may involve home visits of complex patients and liaising with other colleagues in the acute hospital, community teams and primary care. The post holder will provide presence at the team huddles and hold clinical responsibility for the case load (varies between 10-12 UCRs and 50-70 patients on VW). This is supported by a team that can be made up of GPs, specialty doctors, trainees, ACPs, PAs, nurses and therapists. Emergency Department (ED) – provision of clinical leadership within with Front Door Team. The post holder will be supported by the MDT, including junior doctors with the expectation that they will review approximately 4 new patients per day. There is also an expectation to in-reach to other medical wards to support early discharge of patients that could be managed on the virtual ward. Mary Seacole Unit - The post holder will provide leadership for junior colleagues as well as all other members of the multi-disciplinary team (MDT) present on the ward.

Detailed job description and main responsibilities

The principal duty of this post is to provide a comprehensive clinical service to patients.

To at all times, comply with the GMC’s guidance on ‘Good Medical Practice’ as amended or substituted from time to time. To ensure that patient confidentiality is maintained at all times. To undertake duties as defined within the attached job plan in so far as practicable and in agreement with your Clinical Director. To be responsible and managerially accountable for the reasonable and effective use of Trust resources that you use and influence, and professionally accountable to the Clinical Director.

Qualifications and Training

Essential criteria

Hold a medical qualification Appropriate higher medical qualification i.e. MRCP or equivalent. Entry onto the Specialist Register for GMC for GIM and Geriatric Medicine or be within six months of obtaining CCT Entry on the GMC specialist register via CCT, CESR & EU Community Rights

Desirable criteria

MD, PhD or equivalent higher degree or evidence of relevant research.

Research Experience

Essential criteria

Ability to contribute to change management. Ability to apply research findings to clinical problems Ability to critically review research.

Desirable criteria

Publications in peer review journals.

Clinical Experience

Essential criteria

High standard of clinical skill and expertise in the specialty. Experience of working within a community geriatrics service. Comprehensive experience in managing patients with all aspects of geriatric and general medical problems. Experience in leading a multidisciplinary team. Experience of acute medicine, including leading acute unselected medical takes.

Desirable criteria

Experience of service development.

Aware of national policies, quality initiatives

Essential criteria

Understanding of national policies relevant to acute and community care of elderly patients.

Desirable criteria

Evidence of participation in quality initiatives relating to the care of elderly patients.

Management & Administrative Experience

Essential criteria

Audit/Research Has attended a recognised management training event or will have such training on completion of CCT. Supervision of post graduate trainees. Personal time management skills. Clinical Governance.

Desirable criteria

Familiarity within issues of service organisation and development.

Teaching Experience

Essential criteria

Experience of teaching undergraduate and postgraduate trainees. Ability to teach clinical skills.

Desirable criteria

Experience of teaching basic clinical skills to undergraduates / post graduates. Experience of supervising junior doctors. Experience of teaching Lay groups and other staff groups. Ability to supervise postgraduate research. Educational supervision accreditation.
