The role is office based in Cupar County Buildings, Cupar and covers North East Fife Locality within Care at Home Service.
The role is focused on helping people achieve the things in life that really matter to them. You will provide support and assistance to Home Care Coordinators, Lead Officers and Team Managers, supporting them to monitor the quality of the service delivered. You will assist with the day to day running and managing of care packages, recording, monitoring staff training and liaising with other professionals.
You will be trained to take duty whereby you will first point of contact for staff, service users and other professionals.
The below list is an example, not fully inclusive, of responsibilities:
Undertaking assessment of needs and risks as directed. Reducing levels of risk to service users and others. Contributing to and developing care/action plans. Maintaining service user/carer contact as required. Promoting/maintaining independence and inclusion. Engaging and supporting families and carers. Working in collaboration with partners . Health Service, Education, Police and voluntary sector agencies. Improving the outcomes for service users by applying knowledge and judgement. Promoting the interests of service users and carers in terms of rights, opportunities, culture and diversity. Provide a duty service across the locality and other localities if required. Making sure that risk assessment and risk management are central to achieving good outcomes. Ensuring that all interventions and engagement with service users contribute to improving outcomes. Being responsible for the effective application of the electronic client information system by updating and ensuring the accuracy of all relevant client records. Liaise with service user, Home Carer, Homecare Coordinator and other partner services. Any other duties required.The Person
Applicants should have the ability to support a regular and effective service and are also expected to possess effective interpersonal and organisational skills and have the ability travel throughout Fife as and when required, working in partnership to deliver high quality services to service users through robust assessment and case management.
The successful candidates should be able to embrace technology as the role requires the use of various systems to work effectively. You should have the ability to work well within a team and have excellent communication skills. The ability to engage with others and form part of a multi-disciplinary team to achieve common goals and positive outcomes for service users and staff.
Throughout your employment you will undertake a programme of continuous professional development and be willing to undertake continuous training throughout the role.
Before confirming to your appointment, you will be required to obtain Protection of Vulnerable Groups (PVG) scheme membership through Disclosure Scotland and become a member of the relevant PVG scheme.
