Having recently completed a transformation within Adult Social Care in Bracknell, we are now in a position to offer a variety of vacancies in our new structure.

We have vacancies for experienced and motivated Senior Social Workers in our Hospital Discharge Team, which consists of two different functions facilitating the discharges from acute & community hospitals and completing the reviews post-discharge as an opportunity/expectation to rotate between the 2 functions every 6 months.
