  1. Social Worker (Experienced - Adults) 5+ years experience (Paye £28.67 ph (£ % holiday entitlement) / Umbrella £31.92 ph)
Undertake evaluations, formulate care and support strategies, and review and monitor these plans according to the obligations outlined in the Care Act and other applicable legal frameworks. Collaborate with the team to fulfil designated duties. Utilise a strengths-based approach by implementing the conversations model to connect individuals with their social networks and address issues collectively. Work in partnership with individuals, including advocates and caregivers, to devise person-centred plans that prioritise independence, choice, and control while adhering to legal and financial constraints.

Collaborate with family caregivers to identify their needs and develop tailored plans to address those needs. When required, complete the Resource Allocation System and present support plans for approval according to the Scheme of Delegations. Provide support for the effective implementation of these plans. Ensure that all departmental policies and procedures are adhered to throughout the process. Participate in safeguarding assessments and planning.

Attend Multi-Disciplinary meetings, connecting individuals and their families with their communities and social networks to enable them to live independently in their own environments. Act in the best interests of individuals who lack mental capacity, utilising the principles outlined in the Mental Capacity Act to ensure their safety until their capacity returns or alternative arrangements are established.

Requirements
  • Must possess a minimum of two years of UK experience as a qualified social worker.
  • Must have post-qualified experience of a minimum of two years.
  • A valid Social Work England (SWE) Number
  • Professional qualifications in Social Work such as a Diploma in Social Work, BA/BSc/MA in Social Work, or CQSW, as required by Social Work England, are mandatory.
  • The candidate must be eligible to work in the UK.
  • A minimum of five years of references are necessary.
  • An Enhanced DBS (registered to update service/ new DBS required) is essential.
