This is underpinned by a masterslevel that encompasses the four pillars of clinical practice, leadership andmanagement, education, and research, with demonstration of core capabilitiesand area specific clinical competence. Clinical support and supervision areavailable for all staff.

Advanced clinical practiceembodies the ability to manage clinical care in partnership with individuals,families, and carers. It includes the analysis and synthesis of complexproblems across a range of settings, enabling innovative solutions to enhancepeoples experience and improve outcomes.

Main duties of the job

You will be able to expertlymanage urgent community referrals, from our GPs, Hospital and Home andcommunity nurse colleagues. You will manage these cohort(s) of patients andpresenting complaints, providing care for the presenting patient from initialhistory taking, clinical assessment, diagnosis, treatment, and evaluation ofcare, demonstrating safe, competent clinical decision-making and expert careevidence-based care.

To be a clinical expert andsenior clinician in the field of Adult Community Care. To work with the seniorleadership, ACPs and wider team to continually improve the quality of careprovided including the continuation of service improvement to demonstratebetter outcomes for our patients.

The successful applicant willneed to share the values of Mid Yorkshire Teaching Trust and work as part of alarge multi-disciplinary team including GPs, Hospital specialty teams,Community Nurses, Clinical Pharmacists, Allied Health Professionals andAdministrators.

Job responsibilities

The Advanced Clinical Practitionerwill:

Manage their own caseload of patients whopresent with undifferentiated and undiagnosed illness. Undertake audit within the clinical setting todevelop services. Provide formal and informal teaching, support,and clinical supervision to nursing staff and junior medical staff. Ensure maintenance of clinical excellence. Support the wider multidisciplinary team, undertakingindependent comprehensive assessment and management of patients to incorporatephysiological, psychological, and pharmacological considerations, makingmultifaceted complex clinical decisions regarding patient management andclinical outcomes, including, where appropriate, the discharging patients fromthe service when treatment is complete/optimum outcome is achieved.

Key Responsibilities

To be competentin the assessment and treatment of patients presenting with a range ofundifferentiated and undiagnosed conditions underpinned by extensive knowledgeof a range of chronic and acute conditions. To becompetent in providing an initial clinical diagnosis and in the decision torefer to an appropriate speciality. To becompetent in the ability to discharge patients following appropriate assessmentand treatment. To becompetent in the initial immediate assessment and treatment of patients with arange of life-threatening presentations such as cardiac arrest and acute majorillness. V300programme Supplementary and Independent Prescriber, you will be a safe andcompetent independent and supplementary (non-medical) prescriber of medicines(from the British National Formulary). To be ableto see, treat and refer appropriately, a broad range of patients with acute andchronic illnesses as an autonomous practitioner. Administrationof medicines, in line with NMC Guidance and Trust Policy. Demonstrate own personal development with self-directedlearning and identifying and accessing internal and external developmentopportunities, as per individual advanced practice portfolio. Autonomous decision-making including judgmentsinvolving complex facts or situations which require the analysis,interpretation, and comparison of a range of options in relation to the care/management needs of the patients on a day-to-day basis within the clinicalenvironment. Conduct all system clinical assessments,examinations, and investigations to identify abnormalities and make diagnosesand administer a comprehensive and appropriate level of care. Use of advanced clinical skills and autonomousjudgement to prescribe drug treatments for patients following interpretation ofclinical findings through requested bloods and investigations across severalbroad and varied clinical fields. Use best practice, analyse evidence-basedpractice, use of benchmarking to inform clinical practice. Provide detailed and transparent reports ofworkload, outcomes, and recommendations for future developments; demonstratedin advanced practice portfolio this should include case-based discussions,multi-source feedback, etc. Be accountable for the direct delivery andimplementation of clinical care on a day-to-day basis across the full spectrumof illnesses and therapy management. Provide personal care plans to patients anddevelop, implement, and evaluate care including independently prescribedmedications, monitoring therapeutic programmes, and providing health educationand promotion. Be proactive in developing new ways of working,such as ACP led initiatives to increase the quality of the patient journey. Provide health education and advice to patients. Be a safe and competent non-MedicalPrescriber-providing responsible prescribing and adapting medication therapiesto meet the needs of patients. Communication Demonstrate the skills required to communicate, establish,and maintain therapeutic and professional relationships and gain cooperationfrom others. Demonstrate the ability to provide informationthat requires detailed or involved explanation including the balancedcommunication of risk and benefits of tests and alternatives.
