You will empower and educate patients, community members andunpaid carers, to better understand and self-manage their symptoms, to enablepatients to reconnect / stay connected to occupations that are important tothem.

You will be working aspart of our rehab and wellbeing team, predominantly within our Inpatient Unit,but also supporting our community services, and carry out one to oneinterventions and group interventions.

You must be a practicingHCPC registered Occupational Therapist, with a minimum of 18 monthspost-graduation experience in a variety of clinical areas.

Main duties of the job

The Band 6Occupational Therapist will provideOccupational Therapy assessment, treatment, and advice, which especiallyfocuses on wellbeing, self-management, independence, and confidence forpatients with a wide range of life limiting, palliative diagnosis.

The Occupational Therapist will work within themulti-disciplinary team (MDT) to meet the needs of patients accessing GardenHouse Hospice Care services, based within the day services (Rehab andWellbeing) team.

To utilise and promote a rehabilitative palliativecare approach to:

Provide Occupational Therapy interventions for patients attending Dayservices, In Patient Unit and the community hubs.

Manage a caseload, using evidence based/patient centred principleswithin physical and psychological medicine to maintain independence and promotewellbeing.

Evidence of continued professional development CPD portfolio Post graduation experience in a range of general clinical settings care of the elderly, neurological rehabilitation, respiratory, community intermediate care Thorough understanding and clinical reasoning skills of equipment provision Ability to manage a complex caseload in various clinical settings Ability to assess patients needs that may be complex and subject to change, and support discharge planning.

Post registration clinical experience specific to working in palliative care rehabilitation. Expert knowledge of the evidence base of palliative care Health and safety training Previous teaching experience and willingness to undertake appropriate specialist training as required
