Job summary

The Hertfordshire Adult Mental Health Community Transformation Programme is implementing new mini teams within Adult Mental Health services to focus support and give more time to care for service users with low intensity psychosis - a new Low Intensity Psychosis Pathway.

This B6 post will be the named nurse lead for the new mini team in a specified Adult Mental Health Service, working closely with the Clinical Nurse Specialist and the MDT of the service to support this new focus.

The Low Intensity Psychosis Pathway seeks to promote the personalised recovery and future care of those service users who are settled in mental health, and have an enduring diagnosis of psychosis, by working in partnership with people with psychosis and their carers to:

offer help, treatment, and care in an atmosphere of hope and optimism

o take time to build supportive and empathic relationships as an essential part of this care.

Main duties of the job

Our successful candidate will:

hold a strong belief in the rights of people with severe and enduring mental health problems to access community assets in their local neighbourhoods provide assessment and specific interventions to meet the needs of service users, their carers and families be a flexible practitioner who enjoys new challenges in meeting the developing needs of the service and our service users have an excellent understanding of the issues people with mental health needs and carry out interventions to a standard that ensures safe and effective care and complies with the NMC Code

Recruitment & Retention Payment

This post attracts the R & R Premia. The payment will be made as follows: £1,500 on commencement of employment and a further £1,500 in arrears at the end of the first six months, followed by £3,000 in arrears at the end of 12 months. Terms & Conditions apply

Job responsibilities

To lead a mini team of staff to oversee the holistic care of service users on the Low Intensity Psychosis Pathway.

To carry a clinical caseload and fulfil the named nurse role.

To undertake comprehensive health needs and risk assessments(including capacity assessments) and develop a care plan in accordance with identified needs establishing specialistcourse of responsibilities/actions based on Recovery Principles.

To carry out joint assessment with other MDT members as required.

To be aware of and comply with the relevant mental health and workrelated legislation and policy at all times.

To provide relevant up to date evidence based information abouttreatment/intervention that enables service users to make informedchoices about their care.

To deliver evidence based care in a timely manner.

To make reactive recommendations as necessary when working alonein the community.

QUALIFICATIONS/ EDUCATION/ TRAINING

Essential

RMN or Dip/BSc (Hons) / MSc in Mental Health Nursing ENB 998/Preceptorship and Mentorship

Desirable

ENB 812/BSc in Specialist Community Nursing Practice (Mental Health) Psychosocial Interventions

PREVIOUS EXPERIENCE

Essential

Experience of working in a range of settings relevant to the post At least 2 years of postregistration experience with at least one year at Band 5 Experience of working in acute mental health inpatient/day care services Experience of working in situations of crisis and emotionally demanding environments. Experience and knowledge of safeguarding process Experience of teaching and training Experience and knowledge of working in a multi-ethnic community Experience of delivering evidence based care

Desirable

Experience of working with acute mental health problems in a community setting Warranted Approved Mental Health Professional

SKILLS/ KNOWLEDGE/ ABILITY/ KNOWLEDGE

Up to date knowledge of relevant legislation, ethical issues, strategic frameworks and current guidelines regarding the range of presenting problems in adult mental health, their implications for both practices and clinical management Professional Code of Conduct Evidence of continuing professional development Clinical risk assessment and management in a community setting.
