Our role can be challenging and is time sensitive in order to support patient flow through the acute Hospital; but it is also rewarding and varied. You will have the opportunity to work in many different departments and wards throughout the acute site, liaising with many different teams and other specialists in order to facilitate the appropriate discharge requirements for the patients that are referred to us. We also follow patients into the community to provide acute Hospital at Home care.

We work closely with our community colleagues and have established ourselves as a link between acute and community care in order to provide seamless transition of care between Hospital and Home or other receiving destination. We are currently a part of developing a Frailty pathway and are involved in many initiatives around length of stay and discharge planning and frequently receive excellent feedback from our patients as well as our professional colleagues.

We require a forward thinking, dynamic, motivated and creative individual to join us. Being able to work as part of a team and under pressure are essential skills that the successful candidate will hold. Experience from both acute and community backgrounds would be preferred, but is not essential. Experience and understanding of the Continuing Healthcare Framework, Mental Capacity Act and Safeguarding would also be an advantage, but training can be provided with regards to these.

Training and support will be given to the successful applicant, including a robust induction period tailored to your needs. We are a friendly, flexible and supportive team, who face challenges together and ensure we support each other through good times and difficult times.

Pay:  The salary advertised for this position reflects the Agenda for Change 4-5 rates, inclusive of a pay award implemented from October 4. If you join before this date, the pay award will be backdated to April 4 or your start date, whichever is later. Until then, remuneration will be based on the 3-4 pay bandings ().
