We are looking for a Clinical Pharmacist to work across 3 GP practices in Central Hampstead within our Primary Care Network on a 2 year fixed term contract. The successful candidate will be hired by the lead practice but will be deployed across the Primary Care Network. About Central Hampstead PCN Central Hampstead PCN is made up of five practices coming together to provide additional services under the National DES and covers a population of approximately 33000 registered patients.

Main duties of the job We are seeking an enthusiastic and forward-thinking clinical pharmacist to join our GP practice network. We are looking for pharmacists to develop and manage all aspects of medicines across the practice network. Working with highly skilled multi-disciplinary teams you will work in a patient facing role based in the surgeries. This is an evolving role with huge scope for development. The post holder will be part of the significant developments in primary care supporting the Network Clinical Director in improving the care of a diverse population of patients with varying care needs. Qualifications:Masters Degree in Pharmacy (MPharm) or equivalent;Registered with GPhC;Member of RPS;Specialist knowledge through a Postgraduate Diploma and qualifications ( clinical, community, therapeutics) or equivalent;Evidence of recent and relevant Continuing Professional Development;Independent prescriber status or a commitment to undertake the course

Job responsibilities

The clinical pharmacist will, as part of a multi-disciplinary team, be in a patient facing role to clinically assess and treat patients using their expert knowledge of medicines for specific disease areas. Within this role they would be prescribers, or will be completing training to become prescribers, and will work alongside the general practice team. In addition they would be expected to take responsibility for the care management of patients with chronic diseases and undertake clinical medication reviews to proactively manage people with complex polypharmacy, especially the elderly, people in care & nursing homes, those with multiple co-morbidities and people with other disabilities. Key accountabilities (not exhaustive): To be part of a multi-skilled primary care team providing patient centred medical services based on patient needs with a focus on continuity of care. Including- working as part of a multi-disciplinary team in a patient facing role to clinically assess and treat patients using their expert knowledge of medicines for specific disease areas. Being a prescriber, or will be completing training to become prescribers, and will work with and alongside the general practice teams. To take up responsibility for the care management of patients with chronic diseases and undertake clinical medication reviews to proactively manage people with complex polypharmacy, especially the elderly, people in care homes, those with multiple co-morbidities (in particular frailty, COPD and asthma) providing specialist expertise in the use of medicines whilst helping to address both the public health and social care needs of patients at the PCNs practice(s) and help in tackling inequalities. To provide leadership on person centred medicines optimisation (including ensuring prescribers in the practice conserve antibiotics in line with local antimicrobial stewardship guidance) and quality improvement, whilst contributing to the quality and outcomes framework (QoF) and enhanced services. Through structured medication reviews, supporting patients to take their medications to get the best from them, reduce waste and promote self-care. Having a leadership role in supporting further integration of general practice with the wider healthcare teams (including community and hospital pharmacy) to help improve patient outcomes, ensure better access to healthcare and help manage general practice workload. Developing relationships and work closely with other pharmacy professionals within and across PCNs and the wider health and social care system. Taking a central role in the clinical aspects of shared care protocols, clinical research with medicines, liaison with specialist pharmacists (including mental health and reduction of inappropriate antipsychotic use in people with a learning disability), liaison with community pharmacists and anticoagulation.
