We provide Specialist School Nursing to nine Special Schools across the Southampton, Hampshire, IOW and Portsmouth and we are looking to recruit a HCSW to work primarily in Mary Rose School, schools we work with provide education to children and young people who have Special Educational Needs and complex health conditions. The Special School Nursing team works collaboratively with children, families, school staff and the wider multi-disciplinary team within the education setting to ensure a supportive and caring learning environment. The nursing team are responsible for ensuring the health needs of the children are met using a family centered approach to assessing, planning, implementing, and evaluating the health needs of the children within a specialist education provision. The role will include teaching and training of school staff, supervision of delegated skills including children and young people with a range of complex care, epilepsy and seizure management, enteral feeding, and candidate must have recent experience of working with children and people with learning disabilities.

Main duties of the job

About us

We have been working with other local NHS Trusts to create a new organisation, bringing together all community, mental health and learning disability services from across Hampshire and Isle of Wight. Subject to approvals, the aim is for the new Trust to be formed by the end of 2024.

Project Fusion is the name for the programme to create the new combined NHS Foundation Trust. Hampshire and Isle of Wight Healthcare NHS Foundation Trust will be bringing the services into a single organisation will result in more consistent care, more equitable access to services irrespective of postcode, and a more sustainable workforce and services. The new organisation will operate locally to ensure services can best meet the needs of different communities.

The new Trust will be comprised of all the services currently provided by Solent NHS Trust and Southern Health NHS Foundation Trust, the community, mental health and learning disability services provided by Isle of Wight NHS Trust and child and adolescent mental health services delivered in parts of Hampshire by Sussex Partnership NHS Foundation Trust.

Colleagues will transfer to the new organisation under Transfer of Undertakings (Protection of Employment) -- known as TUPE on the day the trust is formed. TUPE is a legal process to ensure you transfer out of one organisation and into another, maintaining the terms and conditions of your employment.

The SSN HCSW will:Support the SSN with administration and implementation of the programmes of specialised care for children who attend specific special schools and support with clinics and health specific delegated tasks and skills to enable them to provide supportive care to children with a variety of nursing needs, including long term conditions and complex and multiple needs, life limiting conditions, learning disabilities and those with challenging behaviour and/or autism related diagnoses.Provide access to health promotion, public health programme and specialist nursing programmes alongside their multidisciplinary team (MDT).Provide specialist support under instruction and guidance from their SSN to the wider MDT, children and their families whilst promoting evidence-based practice.Utilise knowledge and expertise to work in collaboration with the special schools to ensure the children attending can access health promotion, public health programme and specialist nursing programmes alongside their multidisciplinary team. The SSN HCSW will support the SSN team in ensuring the health needs of the children are maintained and the education and support of the school staff is a good understanding and/or experience of children with Profound and Multiple Learning disabilities (PMLD) and Severe Learning Disabilities (SLD).Assist in the administration of medication in school as delegated by the SSN team following assessment of competence and medications management in the second checking of Control Drugs in school as delegated by the SSN team following assessment of competence and medications management the special school nurses in provision of essential training (and updates) to staff for all delegated tasks and health promotion activities carried out in school, including staff school staff are trained and advise on best practice guidance regarding medicines management, medicines waste disposal and medicines administration techniques with support from Special school referrals to other professionals where required which includes continence service, CCN service, CPMS, Respite centres & wider allied health professionals (AHPs)Provide advice, support and referrals for children who are aged 14+ to help with transition planning processes to adult services.Provide advice, support and liaison for children transitioning into school from early years settings/home and provide the necessary care planning, training and referrals as required.Provide advice, support and liaison for children transitioning between schools ( primary to secondary, or to new locations) where necessary.Provide routine monitoring of children on the caseload as necessary including height, weight, and clinical observations as dictated by the Childs health needs.Support with desensitisation programmes and/or use of social stories for children & young people to access wider health care interventions with support from Special School nurses.Signpost families to other services and resources as required, supporting their childs any safeguarding concerns in line with trust and national policies and collaboratively with our social care, education and AHPs to provide holistic care for the children and families on the caseload.Signpost and refer families to necessary charities/agencies in relation to their childs health needs DLA, CHC/CCC applications, make a wish, dream flight, holiday havens etc.
