Job summary

Some ofour previous team members have become Team Leaders and we have also been ableto support the development of 2 team members into Advanced ClinicalPractitioners.

Theteams base is in Kidderminster Hospital, but we offer a county wide servicewith the flexibility of working from home or other trust bases around thecounty. Staff are aligned to geographical areas and we have posts available inboth the north and south of the county (to be confirmed at interview).

Main duties of the job

Ourideal candidate will have a specialist interest in dementia care and someexperience in dementia assessment would be desirable, but for the rightcandidate we would support this development. You will play an integral role insupporting patients and their families through a dementia assessment and postdiagnostic support. This will include delivering pre-diagnostic counselling, takinga detailed clinical history, undertaking cognitive testing and whereappropriate formulating a diagnosis with the Consultant Psychiatrist and communicatingthis back to the patient. Following a diagnosis, we offer a time limited periodof post diagnostic intervention which includes supporting with psychologicaladjustment, monitoring dementia medication, offering specialist advice inrelation to the management of symptoms, liaising with other agencies social services and primary care and onward referral/ signposting.

You will work closely with Occupational Therapists,Psychologists, Psychiatrists and support workers to ensure we offer the bestcare possible.

Therewill be opportunities to enhance your skills and knowledge in dementia carethrough in-house and external training, supervision and coaching.
