Job summary

The GladesNeighbourhood Team is a friendly, supportive and hardworking team who arelooking for enthusiastic individuals to join us. We pride ourselves inproviding outstanding care to patients in the Wyre Forest in Worcestershire.The team operates from 07:30am to 18:30pm on a seven-day rota includingweekends and Bank Holidays, various shift patterns are available. Flexibleworking opportunities such as compressed hours are available.

This postis for 30 hours over 4 days out of 7 on a rota basis including weekends &Bank Holidays (30 hours over 3 days out of 7 would also be considered).

We arelooking to recruit a Healthcare Assistant who is innovative, dynamic, and keento join the team in supporting us to deliver high quality care, and to continueour development of integrated care in the community. The role is varied andrewarding.

TheNeighbourhood Team consists of Community Nurses, Senior Nurse Practitioners,Advanced Clinical Practitioners, Occupational Therapists, Physiotherapists,Healthcare Assistants and administrators. GP and social care colleagues workclosely alongside.

Main duties of the job

Whilst onduty you will either be working alone or alongside colleagues to carry outdelegated tasks in line with the Trust core values.

Followingappropriate training and competency assessment, you will carry out specifictasks delegated by the qualified Nurse or Therapist including fitting anddemonstrating the safe use of equipment; administering prescribed medicationsin line with Trust policy; supporting with activities of daily living such aspromoting independence with personal care or domestic tasks for those patientson the intensive support pathway, support colleagues with moving and handlingpatients and other allocated tasks such as venepuncture, wound dressings andrecording observations.

All clinical documentation is recorded on a system called EMIS Community in line with Trust guidelines and must be contemporaneous. Clear, concise and timely communication is key to your role, and this may be required in a verbal or written format to support patient centred care.

Person Specification

Qualifications

Essential

Level 3 qualification ( NVQ 3, A-level) in Care Or Level 2 qualification ( NVQ2, GCSE Grade A-C) in Care and will be expected to undertake the relevant Level 3 qualification on appointment to post Will be expected to meet the Care Certificate standards within 12 weeks of commencing employment with the Trust. I understand that I will be asked to provide original certificates to verify my qualifications at point of offer.

Desirable

Intermediate Skills Programme.

Knowledge

Essential

Basic knowledge of physical / mental health conditions. A level of understanding of rehabilitation.

Additional Criteria

Essential

Willing to undertake further relevant training programmes. Willingness to work flexibly to meet the needs of the service. Ability to meet the travel requirements of the post.

Experience

Essential

Experience of working with older adults in a health/social care setting.

Desirable

Community experience of older adults with physical and/or mental health problems. Experience of working in a rehabilitation or general medical setting. Experience of working and liaising with staff from different disciplines. Experience of medication administration.

Skills and Abilities

Essential

A positive attitude and able to act on feedback. Ability to praise and be supportive to others. Ability to communicate clearly and effectively in English in both verbal and written formats. Ability to use IT confidently and competently in daily work. Aptitude for promoting independence. Ability to cope with difficult/emotionally distressing/ challenging situations.
