Primary Care Network Transformation Manager

Job type: Permanent. Full time. hours per week. Monday - Friday.

Salary: £44,000 to £49,000. Dependent on experience.

Location: The successful candidate must be willing to travel betweek the Arden Primary Care Network Practices. Alcester Health Centre, Arrow Surgery, Budbrooke Medical Centre, Pool Medical Centre, Henley Medical Centre, Lapworth Surgery, Tanworth in Arden Medical Practice

Main duties of the job

The PCN Manager will act as point of expertise, coordinate the successful achievement of all governance requirements, translate national and local policy into meaningful changes, and will support practices to achieve improved outcomes for the local population. They will also liaise with South Warwickshire GP Federation PCN Managers of other PCNs within South Warwickshire.

Job responsibilities

Job Summary:

To play a pivotal role in the delivery of high-quality primary health care within the PCN.

Support the PCN Clinical Director and board to ensure the smooth running of the Network.

Support practice managers in the PCN member practices.

To be involved in the management and development of the Network enabling PCN staff to work more effectively to support the sustainability of general practice services

To be responsible for the production of performance and quality reports within the Network and other Network initiatives.

Working alongside South Warwickshire GP Federation and have an oversight of the Networks' financial systems/PCNs' employed staff /payments to all the practices within the Network.

To represent the PCN in cross organisation meetings when agreed locality / CCG meetings/ICS and other GM wide meetings.

The Job Description is an outline of the tasks, responsibilities and outcomes required of the role. The job holder will carry out any other duties as may reasonable be required by the CD and PCN board members. The following areas of responsibility are an indication of the role, but could be subject to change as the impact of primary care network commissioning evolves.

Key Responsibilities and Duties:

The PCN Manager's areas of responsibility include:

First point of contact for Practice Managers with regard to delivery of the Networks projects/schemes. Manage and deliver contractual requirements on behalf of the Network. Co-ordinate the delivery of enhanced services and other service submissions on behalf of the Network. Work with South Warwickshire GP Federation to support the recruitment, induction, training and directly line manage PCN staff. Collate feedback on behalf of the Network to report to South Warwickshire GP Federation as required. To manage the workflow processes in relation to the above. To circulate important information in a timely manner to Network Leads, Practice Managers, members of constituent practices and other stakeholders. Supporting the CD to provide strategic and clinical leadership to the network, developing and implementing strategic plans, leading and supporting quality improvement and performance across member practices (including professional leadership of the Quality and Outcomes Framework Quality Improvement activity across the network).

Organisational:

Work with the PCN Director and the board for strategic and operational planning processes to support the PCNs' objectives.

Work with South Warwickshire GP Federation to help the delivery of PCN projects and plans

Support the PCN Director and the board with writing business and development plans as required.

Coordinate meetings for the PCN monthly board meetings and ad hoc project meetings as required.

Identify and record risks and issues, developing contingency plans with service and contract leads.

Ensure all relevant national strategies, policies, targets and frameworks are fully implemented.

Ensure compliance with health and safety policies and procedures to reflect current best practice and ensure all staff, contractors, workers and members of the general public discharge them effectively.

Ensure the PCN has a Business Continuity policy that is updated and in which everyone is kept fully aware.

To be the nominated deputy to attend meetings where the Clinical Director is not able to.

Finance:

To monitor and check the accuracy of the any submissions for Enhanced Services, contractual requirements on behalf of the Network and ensure that all claims are submitted on a monthly and quarterly basis.

Work with South Warwickshire GP Federation to ensure that the PCNs physical and financial resources are managed and maintained for maximum effectiveness through.

Work with South Warwickshire GP Federation and be responsible for ensuring payments are received, made, checked and chased for the PCN.

Provide a PCN budget/forecast and share a quarterly financial update to keep members informed.

Digital:

To review and improve the PCNs digital maturity.

To work with South Warwickshire GP Federation and PCN to use data, and improve data quality, to support population health management, improvements in care quality and PCN operational efficiency

To work with South Warwickshire GP Federation and PCN to use data, and improve data quality, to understand the type and intensity of support needs of their PCN and coordinate this support. To ensure that digital and operational PCN transformation is embedded in, and aligned with, ICS and national strategies.

To work with SWGP South Warwickshire GP Federation to support the adoption of new technology and other initiatives that aim to improve the quality of care offered by the PCN

IM&T:

Ensure the PCN's compliance with the GDPR and FOI along with other relevant legislation and professional information management standards such as GMC/BMA and LMC guidance.

To oversee the administration of the PCN.

To ensure there are robust systems and processes in place for the network to operate as efficiently and effectively as possible

Relationship Management:

Manage and maintain credibility with all key contacts within the network, to foster a culture of collaboration, and promote inter-service learning to drive continuous improvement. To work as a flexible member of the PCN providing support to other team members when necessary.

To take an active role in the development and embedding of the PCNs culture, values and reputation as providers of high-quality services.

Encourage supporting staff to collaborate through sharing information and intelligence across different activities.

Develop and support the relationship between the PCN and South Warwickshire GP Federation to ensure PCN needs are being met.

To support a shared identity and branding within the network.

Work with practices within the network to identify areas of common concern or potential for collaborative working to improve the resilience of general practices, sustain local services and support recruitment and retention of key members of the primary care workforce.

Network Member GP practices, Network Board, Clinical Director, key stakeholder partners from NHS, local government, third sector, Network employees, local service users and the population of our network.

To have the ability to gain the confidence and buy-in from member practices to enable their full participation in networks and their delivery.

Act as a key Network representative at senior level.

Support public engagement and consultation on network plans.

Personal/Professional Development:

The post-holder will participate in any training programme implemented by the PCN as part of this employment, such training to include:

Participation in an annual individual performance review, including taking responsibility for maintaining a record of own personal and/or professional development. Taking responsibility for own development, learning and performance and demonstrating skill and activities to others who are undertaking similar work. Training may need to be undertaken outside of normal practice hours, and off site.

Quality and Governance:

The post-holder will strive to maintain quality within the PCN, and will:

Be an expert opinion and advisor upon issues of quality and risk Assess own performance and take accountability for own actions, either directly or under supervision. Work effectively with individuals in other agencies Effectively manage own time, workload and resources Ensure that the PCN board and Clinical director adhere to the governance structure as set out in the network agreement The post-holder will strive to maintain quality within the practice network, and will contribute to the effectiveness of the team by reflecting on own and team activities and making suggestions on ways to improve and enhance the teams performance Develop an appropriate governance framework for the smooth operation of the PCN and the delivery of its plan. This will encompass the establishment of a legal framework between practices, agreed ways of working such as schedule of meetings, regular reporting on progress against the plan, and effective communications and engagement within and external to the network.

Communication:

The PCN Manager will ensure effective communication is maintained within the Network and with all stakeholders outside the networks.

Qualifications

Essential

Educated to Degree level or equivalent experience of working at a senior level within primary care. Evidence of Continuous Professional Development.

Experience

Essential

Have knowledge of GMS contracts. Experience of strategy development and implementation. Experience of primary care and specifically general practice current working knowledge either from time spent working in General Practice or time working to commission/interface with General Practice. Evidence of leading and implementing change/innovation Evidence of leading on the development of business cases, and tender responses. Operational management of multi-disciplinary groups of staff. Experience of project management including budget management and HR deployment and Demonstrates sound judgement in the absence of clear guidelines or precedent, seeking advice as necessary. Strategic thinker with strong analytical skills ability to anticipate and resolve problems before they arise. Experience in analysing, interpreting and presenting highly complex data. Assessing risk and planning mitigation Experience and working knowledge of all relevant and legislative responsibilities relating to GDPR, Health & Safety, Human Resources and CQC. Experience of writing reports and polices. Experienced at working in partnership with senior clinicians and or at a senior level within an organisation.
