We are seeking a self-motivated individual to join our successful service in Southwest London - to help set up the new health and wellbing coaching and social prescribing service at St Georges hospital within the Red Cell Pain Management & Psychology Service.

The Merton Social Prescribing service is one of the most established and mature services in the UK and celebrated its 7th anniversary this year. 

You will use your coaching and motivational interviewing skills and provide a person centred service to patients within secondary care across 5 boughous. You will also support each patient to access appropriate services provided by voluntary and community organisations and other agencies. 

The new health and wellbeing coaching and social prescribing service will expand the options available in Secondary Care by creating a referral pathway of patients experiencing social, emotional and/or practical needs. Patients can be linked to a variety of local non-clinical services in the community. These include leisure, social activities, education, welfare, housing and employment advice and support. 

About The Role | How to Apply: 

  • 21 hrs per week - 3 days  - 9 - 5 pm - Monday Wednesday Friday

  • 1 year fixed term contract Pilot (with extension for year two)

  • £ 32,964 per annum pro-rata - plus 5% pension, includes Outer London Weighting, generous annual leave and excellent training opportunities
