This is a fantastic opportunity for a passionate, motivated member of staff to develop extended skills and knowledge in pressure ulcer prevention, complex wound management and patient safety improvement initiatives. As the service forms part of the corporate nursing directorate the team are involved in many quality improvement project and initiatives working with the senior leadership team across the Trust.

The tissue viability service provides subject matter expert advice to all clinical areas, so the variety of people you will meet is huge. Even though we are a strong team you will work autonomously, making decisions every day, doing what you need to do and giving advice to the wider team on the best ways to manage patient’s wounds, alongside providing teaching and education to all staff.

Main duties of the job

You will play a key role in supporting the Tissue Viability Lead Nurse and the Matron for Patient Safety and Safe Staffing in providing a comprehensive evidence based tissue viability service incorporating, advice, education and training for both patients and health care professionals at Lancashire Teaching Hospitals (LTHTR). 

Have specialist knowledge of tissue viability, expected to work autonomously to clinical competence and within scope of practice. Being responsible for ensuring the care given to patients for the prevention and treatment of complex wounds and pressure ulcers meeting the individual needs of patients as well as meeting national standards, research based best practice and local standards. while monitoring the incidence and prevalence of pressure ulcers in the Trust and the use of pressure relieving equipment.

Provide clinical advice and support for staff and contribution to education and training of all disciplines in tissue viability. The role will involve working directly with the clinical teams to provide effective patient care, including monitoring standards of care and record keeping as well as contributing to audits.

Will be expected to develop professional links both internal and external agencies including community tissue viability teams and district/community nurses. They will be expected to establish good working relationships with all members of the multi-disciplinary team, providing expert clinical support throughout the Trust.

Detailed job description and main responsibilities

Assessment and care planning to meet the health and wellbeing of patients Demonstrate a high level of complex decision making in relation to the assessment, planning, intervention and evaluation of individualised patient care. Provision of care to meet health and wellbeing needs of patients. Demonstrate a high level of care provision. Communicate effectively using appropriate methods with people at all levels both inside and outside the organisation to ensure seamless care. Maintain and develop your own professional competence and seeks to extend scope of personal knowledge and skills within the scope and role of professional practice. Be aware of the Health and Safety at Work Act and all Trust policies and procedures in relation to health and safety to ensure agreed safety procedures are carried out to maintain a safe environment for everyone. Supports and assists the matron for patient safety and safe staffing in the promotion, development and sustainability of a patient safety culture. Undertakes a wide range of interventions in order to support the patient safety team in the delivery of patient safety initiatives, enabling the Trust to deliver sustainable improvements care. Contribute to high quality patient safety by utilising improvement methodology to support our clinical teams in improving the quality of care given to patients across the Trust. Facilitate and monitor progress of relevant patient safety improvement initiatives. To ensure that work is completed within timescales, meeting the required quality standards e.g. level of service is provided, thoroughness, adherence to quality measures and protocols. To support equality and diversity in relation to the actions and responsibilities of everyone. To participate in the design and implementation of a structured audit programme. Gather, analyse and report a wide range of data and information. Disseminate knowledge and skills to all staff across the Trust to enable ward teams to take forward service improvement initiatives.

First level Registered Nurse Mentorship qualification or equivalent

Knowledge & Experience

Significant post-registration experience (2years+) Broad clinical experience in a clinical setting Experience of assessing and managing pressure ulcers, complex wounds, leg ulcers Experience of implementing change within the nursing environment Experience of leadership and management in a clinical area Clear understanding of current tissue viability best practice and national guidelines Proven ability to successfully implement, maintain and audit policies, procedures and documentation Evidence of continuing professional practice Knowledge of risk management/health and safety

Desirable criteria

Participated in clinical audit Experience of patient safety improvement methodology (eg model for improvement) Experience of undertaking research to advance the field of tissue viability

Skills and Abilities

Essential criteria

Up to date knowledge in the management of complex wounds and pressure ulcer management The application of topical negative pressure (TNP) systems. The application and management of larvae therapy. Evidence of teaching and facilitating staff development in the area of tissue viability. Excellent communication and interpersonal skills. Able to support and facilitate service development. Able to work within a team and on their own initiative. Demonstrates a commitment to multi-professional learning. Computer skills, ability to use word, excel, power-point.
