Job summary

We are looking for a creative, pragmatic, and organised person who enjoys working with people in the community. This successful candidate will play a crucial role in our community speech and language therapy team, taking a lead on our service delivery to care homes across Sheffield. This post is Band 7, 30 hours ( wte) per week, and is a permanent role.

This post provides speech and language therapy as part of the SLT Adult Community Team which provides an SLT service for people living in care homes and their own homes. You will be supporting clients with communication and/or swallowing difficulties, often with multiple pathologies. Working with, and via carers is an integral part of our management approach.

The successful candidate would be joining the team during an ongoing project that has a strong focus on collaborative training for care home staff in regards to nutrition and hydration. You would have the significant role of delivering this project, working alongside the team leader and team members to implement and embed practices into everyday working.

You will have extensive experience of working in the primary and community settings and in particular working effectively with other AHPs, nursing and care staff roles to provide the best patient centred care. Our team has a strong networking ethos. There are strong links with the hospital SLT service and other community services.

Main duties of the job

To provide highly specialist Speech and Language assessment, therapy and support to a highly complex caseload of adults referred to the Adult Community Team. To provide highly specialist Speech and Language Therapy input as part of a team. The post holder will specialise in the management of patients with highly complex dysphagia and communication impairments. To provide clinical leadership for the care home service delivery within the Adult Community Speech & Language Therapy Team

About us

The SLT service across both the acute and community teams in STH has approximately 60 staff (49 therapists, 7 assistants and 4 secretarial staff).

The SLT teams are highly motivated to provide a quality service to their patients, they are research active and embrace innovation. The service has established processes for supervision and supports personal and professional development.

There is a strong clinical support network in Sheffield. All therapists have regular clinical supervision. There are monthly clinical meetings and access to clinical lead specialists including AAC, Older Adults, Voice and Dysphagia.

Qualifications

Essential

Relevant professional qualification (1st degree level or equivalent qualification/experience) Post graduate training in dysphagia management Registered member of Royal College of Speech & Language Therapists and Health Care Professions Council Evidence of highly specialised continuing professional development Evidence of successful completion of post graduate specialist short course up to Masters degree level equivalent

Experience

Essential

Extensive and in-depth experience of relevant clinical area. Experience of managing students including: placement organisation, mentoring, evaluation, feedback Involvement in development and delivery of training packages. Experience of working with a multi-agency, multi-discipline team.

Desirable

Experience of service evaluation including outcome measurement, evaluation, identification of possible funding sources, writing reports/bids Experience of leading on and extensive participation in multi-organisational projects. Extensive experience working in care home settings.

Skills and Knowledge

Essential

Evidence of ability to manage specialist and complex caseload independently. In depth knowledge of broad range of assessment tools and therapeutic interventions for relevant client groups. Knowledge of standardised outcome measures and skills in the development of service-specific outcome measures. In depth specialist knowledge of national policies and legislation relevant to client group. Knowledge and application of professional standards and code of conduct. Competent in use of IT for everyday communication. Knowledge of the principles of clinical governance, audit and required standards of record keeping.

Desirable

Knowledge of primary and / or community health care. Awareness of the Directorate's NHS agendas and supporting health policies

Essential

Excellent communication skills and interpersonal skills, especially in relation to liaison and joint working. Evidence of highly developed negotiation skills and management of conflict. Flexible approach to working practices. Commitment to person centred planning and user and carer involvement in service delivery and development. Valid full driving licence and access to a vehicle which can be used for work purposes.
