Job summary

We are seeking a motivated pharmacist to lead the clinical pharmacy services for the Acute Care Unit at CRH. This encompasses the Emergency Management Unit, Short Stay Unit and Same Day Emergency Care (SDEC). Main duties of the job

You will undertake a lead pharmacist role working closely with Acute Care Unit team on all aspects of Medicines Management. You will work clinically across the unit utilising your independent prescribing skills to optimise patients' medication. You will have a key role in managing clinical pharmacy services at the point of admission and linking with other teams and specialities as the patient moves on their journey through the hospital or back out into the integrated care system.

You will lead and support the development of junior pharmacists and other members of the MDT that rotate through the admissions areas and coordinate the delivery of pharmacy services to these areas. This will include oversight and governance of the EMU satellite pharmacy and ACU automated dispensing cabinets.

Job responsibilities

To lead the provision and development of clinical pharmacy services to the Acute Care Unit and support the Principal pharmacist in the provision of the wider divisional clinical pharmacy services The Pharmacist should: Lead the processing of the Care Unit requests for new medicines Advise on the review of the Medicines Formulary as it applies to the Care Unit Facilitate the development and implementation of medicine-related guidelines Undertake the Pharmacy aspects of the induction of new medical staff in the Care Unit Lead the auditing of medicines in the Care Unit Attend appropriate Acute Care Unit meetings to ensure that pharmaceutical issues are considered Lead the review of medicines related incidents within the Care Unit Monitor and investigate Controlled Drugs (CDs) governance issues within the Care Unit Lead pharmaceutical developments within the Care Unit Monitor interventions in treatment to identify training and information need Provide training to Care Unit staff To provide expert advice and information on new drugs and new treatment strategies in area of responsibility by evaluating and interpreting published evidence To monitor information on drug expenditure/prescribing patterns and advise consultants, senior pharmacy managers and general managers on clinical economy within area of responsibility To monitor compliance and advise on the implementation of National Guidance NICE To advise the Care Unit / speciality management team on service and product developments and to contribute to the planning and implementation of any changes in practice To undertake prescribing within a defined speciality, and as part of wider medicines optimisationinitiatives undertaken by the clinical pharmacy service. To undertake the administration of medicines to patients in defined circumstances To undertake clinical pharmacy practice at an advanced level.
