**Salary**: £50,952- £55,300 depending upon experience (Must hold ACP Badge)

An exciting new opportunity has arisen for a qualified Advanced Clinical Practitioner (ACP) to join our ever-expanding Primary Care team. Royal Primary Care (East and West) form the North Derbyshire PCN with a weighted practice population of 36,000; this role will work within the existing team across the two practice populations.

You will work to provide both pre-arranged and same day visits to housebound patients with chronic and acute conditions and occasional visits to those living in care settings. The role requires the practitioner to be able to assess and treat patients within their scope of practice:
Providing effective care planning and comprehensive assessments

Supporting the development of effective systems and processes to ensure well-coordinated, high-quality care.
