Job summary

RHA has an exciting opportunity for an experienced anddriven individual to join our organisation to support our local primarycare networks and take onthe role of Digital Transformation Manager.

The post holder will be a keymember of the team and the role will include supporting our member practices todeliver the strategic and operational aims of the PCN.

The post-holder will be expected todevelop and establish strong collaborative relationships with practices and allstakeholders. Internally these include practice members of the PCN and RHA and,externally, the CCG, and most importantly, thePCN patients.

As the role of Primary CareNetworks, within the NHS, develops it is envisaged that the demands andrequirements of the post will increase over time.

The job description and person specification are anoutline of the tasks, responsibilities and outcomes required of the role. Thejob holder will carry out any other duties as may reasonably be required by theClinical Director. The following areas of responsibility are an indication of the role butcould be subject to change as theimpact of primary care commissioning emerges.

Main duties of the job

The core objectiveof the PCN Digital and Transformation Manager role is to support the PCN andits practices to deliver ongoing improvement to services.

Toplay a pivotal role in the deliveryof high quality primary health carewithin the Primary Care Network (PCN).

To developstrategy related to transformation and improvement.

Support the PCN Clinical Directorand the member practices to ensure the smoothrunning of the PCN.

To support the production ofperformance and quality reports within the PCNand other Network initiatives, liaising with other stakeholders as required.

To be able tointerpret large quantities of data and disseminate this to relevant parties.

About us

Rochdale Health Alliance was established in 2016 by GP practices from across the Rochdale Borough to streamline the way in which services are delivered. Each of the 3 areas of the borough elected GPs to represent their locality and become directors on the board. The board is supported by a management team who ensure the day to day functioning of the organisation.

GP practices across the Rochdale borough, who have historically worked independently, have came together to work in a federated model to improve healthcare across the Heywood, Middleton and Rochdale (HMR) neighbourhoods.

Job responsibilities

Key Responsibilities and Duties:

First point of contact for PCNPractice Managers regarding delivery of PCN projects/schemes.

Lead on ITprojects.

Createdashboards for relevant PCN data to be delivered in stakeholder meetings.

Support the induction of new PCN staff.

Management and administrative supportof monthly meetings.

Work alongside practices tounderstand current challenges to effective service delivery and identifysolutions, including existing best practice within the PCN.

Recording and management of holiday and sickness of PCN staff.

The post holder will alsodevelop relationships with local delivery partners (such as IT serviceproviders and other NHS organisations) to support effective delivery oftransformation.

Work alongside practices tounderstand current challenges to effective service delivery and identifysolutions.

Build relationships across the PCN to developconsensus for improvements and to ensure business and clinical staff are fullyengaged.

Support staff to identify and participate inrelevant training and/or professional development opportunities.

Build and maintain good working relationships with abroad range of internal and external stakeholders.

The post holder will be responsible for thedevelopment, management and maintenance of systems and processes to ensure thePCN operates at the highest level of performance and accountability.

The PCN Digital and Transformation Manager will forman integral part of the PCN leadership working alongside the Clinical Directorand PCN member practice colleagues.

The post holder will act as the main point ofcontact of expertise, coordinate the successful achievement of all PCN contractualrequirements, support the translation of national local policies into meaningfulactions, and support collaborative working to achieve improved outcomes for thelocal population.

Analyse service specification, performanceindicators and data collection requirements to inform design for new PCN services.

Collate feedback/analysis data on behalf of the PCNto report as required.

Help develop joint working and relationships acrossnetworks and its stakeholders.

Carry out any other duties as may reasonably berequired by the Clinical Director.

Essential

Degree or equivalent level education OR ability to demonstrate competency at this level

Project Management Certificate (Quality Service Improvement and Redesign (QSIR) or Prince 2)

Demonstrable experience of general practice and primary care Demonstrable experience in successfully supporting delivery of new initiatives Knowledge of budget management Experience of developing and managing relationships with a variety of clients, stakeholders
