We are a large forward-thinking paperlessPractice of 17,000 patients, based in excellent modern purpose-builtpremises. Our strength lies in thediversity of skills and professional interests within our team. We have amultidisciplinary clinical team made up of Doctors, Advanced NursePractitioners, Pharmacists, First Contact Physios, Health Care Assistants,nurse Associate, Mental Health Practitioner, Health and Wellbeing Coach andSocial Prescribers. We are committed to facilitating ongoing continuingprofessional development, we offer structured education sessions and regularclinical supervision sessions with GP mentor which is built into the rota.

Main duties of the job

In this roleyou will work independently, making professional, autonomous decisions.

You will be responsible for making necessaryreferrals, requests for investigations and do these in an appropriate mannerand timescale.

The role will involve initial history taking,clinical assessment, diagnosis, treatment plan and evaluation pf patient care.

The role will involve independent prescribing withthe scope of professional practice

The advanced nurse practitioner will need to proprietiesand triage the needs of patients accordingly, seeking GP input as appropriatewhere the problem is outside their professional capability.

You will demonstrate safe clinical decision makingand a high level of independent care including assessment, diagnostic andprescribing skills.

You will attend patients with urgent medicalproblems.
