Job summary

Sherwood Forest Hospitals NHS Foundation Trust is seeking a dedicated and proactive discharge specialist practitioner to join our team, focusing on discharge coordination. The successful candidate will play a pivotal role in ensuring the timely and effective discharge of patients across the Trust. Working closely with ward-based multi-disciplinary teams and the discharge hub, you will:

Ensure the provision of specialist advice. Develop and implement policies to manage the discharge process. Ensure all patients with long lengths of stay over 14 days have clear discharge plans. Conduct Reason to Reside checks for all patients. Participate in the Length of Stay (LOS) report weekly meetings and other relevant meetings.

Experience at a Band 5 or above with specialist skills and knowledge in assessing the needs of patients with complex health and social care needs and extensive experience within medical / care of the elderly nursing. Knowledge and experience of intermediate care services. Evidence of multidisciplinary working at an advanced level.

Knowledge of the legislation for discharge processes In depth knowledge of continuing health and social care policies Policy writing and interpretations Change management skills Recent participation in clinical audit and / or research project
