This is an exciting new post and the start of a development whereby we aim to enhance inpatient care for frail older people, exploring new ways of working within the multi-disciplinary team to enhance inpatient care and discharge from hospital of patients on acute geriatric wards. The postholder will be based on a -bedded ward for the acute and post-acute care of frail older people with complex co-morbidities, dementia and delirium. The postholder would share care of post-acute patients with the medical Consultants, focussing on progressing complex geriatric assessment and ensuring that the ongoing management for the patient is focussed on their goals, whilst effecting safe, timely, well-planned discharge from hospital.

We aim that introduction of this new way of working will free up Consultant Geriatrician time to deliver other Consultant-led activities, demonstrating further benefit to the frail older patients across our population. If this is successful we would aim to duplicate the role in other areas of the specialty, giving scope for a future rotational post.

Applicants from Nursing and Physiotherapy backgrounds who are passionate about the holistic, multidisciplinary care of frail older people are welcomed and encouraged to apply. Prescribing/deprescribing would be an essential part of the role so the post is unlikely to suit individuals with an Occupational Therapy background.

The post advertised is for an ACP who has already completed the relevant training in advanced clinical practice, although individuals interested in undergoing this training whilst in post are also encouraged to apply. Therefore, applications are welcome from both qualified and unqualified candidates.

Working Pattern: per week

Pay: The salary advertised for this position reflects the Agenda for Change 4-5 rates, inclusive of a pay award implemented from October 4. If you join before this date, the pay award will be backdated to April 4 or your start date, whichever is later. Until then, remuneration will be based on the 3-4 pay bandings (Pay scales for 3/ | NHS Employers).
